COR-040000 Stormwater Management Plan <br />?, • Identification of erosion and sediment controls, equipment, and site areas with high <br />pollution potential (chemical or equipment storage areas) that should be inspected on a <br />monthly basis during active mining operations. <br />• Visual inspection of stormwater management devices, such as erosion and sediment <br />controls, equipment, and storage areas monthly during active mining operations. <br />• Inspection and testing of equipment and systems as needed to prevent conditions that <br />could cause breakdowns or failures resulting in discharges of pollutants to surface waters. <br />• Appropriate and timely maintenance, repair, or replacement of control measures and <br />equipment. <br />• Routine inspections will be conducted for leaks or conditions that could lead to <br />discharges of pollutants or contact of stormwater with pollutants. <br />• Preparation of thorough records for inspections of equipment and systems. <br />• Maintenance of a recordkeeping system of mining activities with respect to the SWMP. <br />The following list of activities and information should be recorded in the SWMP: <br />o A record of spills, leaks, or overflows, including time and date, weather <br />conditions, and related factors; <br />o Implementation of specific items in the SWMP and stormwater site plans; <br />• o Training events (given or attended); <br />o Events involving material storage and handling; <br />o Contacts with regulatory agencies and personnel; and <br />o Notes of employee activities, contacts, and notifications. <br />Additional information, such as dated photographs, field notebooks, and figures, may be <br />included where appropriate. Denison is responsible for informing any subcontractors of <br />this Plan and ensuring implementation and compliance. Appendix C provides a spill and <br />leak reporting form and log. Appendix E provides an example inspection form for <br />documentation purposes. Visual inspections will occur monthly during active mining <br />operations and Comprehensive Site Evaluations will be performed semi-annually as <br />described in Section 5. <br />An inspection checklist that may be used when conducting preventative maintenance <br />inspections is included in Appendix E of this SWMP. <br />d) Good Housekeeping (COR-040000, §I.B.4.d): <br />Good housekeeping practices are encouraged at the Van 4 Shaft site and are implemented to <br />maintain a clean and orderly work environment. Good housekeeping is an effective first step <br />toward preventing accidental spills of materials that may have an adverse impact on <br />stormwater quality. Clean and orderly work areas reduce the likelihood of accidental spills <br />caused by mishandling of materials and equipment, and reduce safety hazards to facility <br />personnel. <br />Van 4 Shaft SWMP 9 Denison Mines (USA) Corp.
