• within the work area. The employee records the task being performed, observation(s) of unsafe
practice, and suggested actions to mitigate the unsafe practice on a AMSO BBS form (Appendix
A). BBS Observation Forms will be kept readily available at each work site, and completed
forms will be submitted directly to the SSHO or designee, who is responsible for maintaining
completed forms. The information provided on the form is shared with the project team during
the following daily tailgate safety meeting. Tlie SSHO adds the feedback/input provided by the
project team, documents the actions taken to mitigate the unsafe practice, and provides the
follow-up information needed to assess whether the actions successfully mitigated the unsafe
practice.
The information provided on the forms is also added to the AMSO BBS database where the
information is used to update the SHP as necessary and track observed at-risk behaviors at
project sites. The information compiled in the database is reviewed to identify any trends or
potential trends in at-risk behavior. Potential trends are monitored to determine if actual trends
are suggested in the data. Once a trend is identified, an action plan is developed to address the
at-risk behavior or practice.
i The AMSO BBS Observation Process provides a constructive tool for maintaining and
continuously improving a safe work environment. The objectives of the process are:
Demonstrate that an actionable process is in place for continuous safety improvement.
Ensure employees are properly trained and encouraged to observe each others actions,
recognize and properly document unsafe practices, openly provide feedback/input
regarding corrective measures.
Track at-risk behaviors observed at AMSO work sites and update the SHP as necessary.
Monitor for trends in at-risk behaviors and develop action plans to address any
identified trends.
Ensure actions are tracked through to completion.
3.1.4 Chronic Safety Offenders
Anonymity will be enforced for first-time offenders. However, if an employee exhibits serious .
and chronic safety issues, coworkers are encouraged to notify the project supervisor of their
observation before any other actions are taken.
