MODIFICATION 03-2009 <br />Exhibit C - AMSO, LLC Notice of Intent Supplemental Information <br />Is Proposed Drill Pads <br />Modifications to the drill pads specified in the approved NOI include the construction of <br />one new pad (TM), reduction of the size of drill pad HB, and minor re-location and <br />reduction of the size of drill pad MWP-2. The drill pads will be constricted at the <br />locations shown in Modification 03-2009 Exhibit A (Figures A.1 and A.2). <br />Drill Pad TM will be constructed east of the existing drill pad TP, near the intersection of <br />the existing BLM road and the existing primitive road. The pad will be used to construct <br />one new exploration borehole (BH-1) and the 4 tomography wells included in the <br />approved NOI. The 4 tomography wells were originally to be drilled on drill pad TP, but <br />are being moved to drill pad TM tinder this modification to the approved NOI. Drill pad <br />TM will be constructed as shown in Modification 03-2009 Exhibit D (Exhibit D.7). All <br />fill areas will be properly compacted to minunize the potential for subsidence or slope <br />failure. To reduce erosion and soil loss, ditches and berms will be constricted to divert <br />stormwater away from the pad. Silt fencing and/or other sediment retention measures <br />will be used to reduce sediment migration from the disturbed area. The disturbed area for <br />the drill pad TM will be approximately 1.5 acres, including a 200 feet by 160 feet pad <br />and associated soil stock pile. <br />The size of drill pad HB will be reduced to the dimensions shown in Modification 03- <br />2009 Exhibit D (Exhibit D.3). The disturbed area for the modified TP drill pad will be <br />approximately 1.4 acres, including a 160 feet by 130 feet pad and associated soil <br />stockpile. <br />The location and size of drill pad MWP-2 will be modified as shown in Modification 03- <br />2009 Exhibit D (Exhibit D.5). The disturbed area for the modified MWP-2 Pad will be <br />approximately 1.1 acres, including a 160 feet by 130 feet pad and associated soil <br />stockpile. <br />A containment pit was constructed on drill pad TP as specified in the approved NOI. <br />AMSO does not intend to constrict containment pits on drill pads TM, HB, or MWP-2. <br />If the need for a containment pit on any pad is warranted in the future, the pit will be <br />constricted as specified in the approved NOI. <br />Waste Containment and Disposal <br />O CHANGE <br />Reclamation <br />NO CHANGE <br />6 Proposed Disturbance (approximate) Describe the proposed drilling to be conducted, <br />• including anticipated number of holes, diameter, depth, location, etc...
