1) To document unweathered nahcoloitic oil shale faces, via photography and/or laser <br />mapping. <br />2) To remove several large rock samples from the existing mine workings for fiirther study. <br />An initial video inspection will be performed to gauge the condition of the shaft and entrances <br />to the various mine sub-levels. This will require orderly removal of the existing concrete cap <br />on the vertical mineshaft to perform the video inspection and possibly a preliminary <br />reconnaissance using a manned inspection capsule lowered via portable hoist or crane. <br />To access the mine levels, a hoisting system (consisting of a temporary steel headframe <br />structure, hoist, and cage designed and manufactured to fit the existing vertical shaft guides) <br />will be erected. <br />Because the shaft will be accessible during this work, a series of safety and security measures <br />will be implemented to prevent access to the mineshaft by unauthorized personnel. <br />6E. Indicate Chemicals and Fuels used or stored on site. List type, quantity and method to store. <br />Fuel will be stored in a skid-mounted, double-walled fuel tank within a lined containment area. <br />Liners will be placed under stationary diesel equipment to contain any oil, lubricant, or fuel <br />drips. Refueling or oiling of mobile construction equipment will be performed with a <br />temporary liner laid out between the equipment and the fuel or lubricant supply tank. <br />Unattended refueling will not be permitted. Any soil affected by a fuel or hydraulic oil leak will <br />be excavated and removed to a legal offsite disposal facility. A spill containment and <br />collection kit will be present at all times. <br />CONTINUED FROM PAGE 5 OFNOI FORM <br />6L Describe and locate any structures to be constructed (i.e., stockpiles, ponds, impoundments): <br />Temporary work facilities will be located onsite for use during mine reentry and sampling <br />operations. No permanent structures or facilities are planned. Temporary facilities are <br />depicted on the attached diagram entitled "Horse Draw Mine = Proposed Equipment Layout <br />and include office trailers, fuel tank, water tank, storage containers, scrap metal container, <br />refuse container, and portable toilets. Woody vegetation and suitable plant growth material <br />will be removed from the proposed work area and stockpiled. Plant growth material stockpiles <br />will be covered with erosion control material. <br />IV. Operation and Reclamation Measures <br />CONTINUED FROM PAGE 6 OFNOI FORM <br />5. Describe how portals, adits, shafts, ponds, excavations, or other disturbances will be reclaimed <br />(refer to Rule 3 and Rule 5 for specific reclamation performance standards). You may wish to <br />contact the Division for closure specifications. <br />Once the existing concrete cap has been removed, unless the project is to be abandoned after <br />initial inspection, a temporary steel bar grating deck supported by steel beams will be installed <br />over the shaft collar. After completion of project activities, the temporary grated steel deck <br />will be removed and the shaft closed by installing a more permanent segmental pre-cast <br />concrete cap over the concrete shaft collar similar to the existing concrete cap. <br />11
