CONTINUED FROMPAGE 4 OF NOI FORM 2 (Public File) <br />III. Proiect Description <br />5. Describe proposed surface excavation or other land disturbance, including roads, pits, trenches, <br />waste piles, drill pads and collar areas of underground workings, ponds, etc.... <br />Surface disturbances will be limited to a portion of the existing disturbed area on the mine <br />bench and the existing unpaved road leading to the mine. <br />The existing concrete cap on the vertical mineshaft will be removed in an orderly fashion so the <br />mine can be accessed to complete this work. A temporary grated steel bar deck or concrete <br />cap will be placed on the shaft between phases of the project. <br />Work in the collar area may include re-grading and possible addition of fill material to raise <br />grade around the shaft collar to accommodate installation of a temporary mine hoist to access <br />the mine. This hoist system may require the pouring of a temporary concrete slab and/or <br />footers. Should these be necessary, they will be constructed with embedded liming slings to <br />allow them to be easily removed from the site upon completion of this work. <br />Within the existing chain link fenced area, at the shaft collar, a second area will be fenced off <br />using plastic construction fencing and steel t-posts, with appropriate signage. This small area <br />will control access to the shaft collar while the cap is off, and fall protection measures will be <br />required within this area at all times until either temporary steel bar grating deck supported by <br />steel beams has been installed, or a temporary concrete cap is re-installed. <br />Erosion control measures will be employed for any areas of significant earthwork. Storm water <br />runoff will be controlled per state regulations. <br />The existing collar area will be accessed via existing roads. The unpaved access road to the <br />mine may need to be repaired to ensure adequate safety, and this may require bringing in fill <br />material or gravel to fill in potholes and provide proper drainage. Culverts will only be <br />installed if deemed necessary to keep the road open during this work. <br />Prior to mobilization to the site, ground-engaging equipment will be pressure washed to remove <br />any remaining earthen material that may contain weed seeds. <br />The site will be kept clear of any loose trash that could blow into adjacent areas. All waste <br />material and construction debris will be placed in appropriate containers kept on site and <br />disposed of in accordance with Federal, State, and County regulations at an offshe location. <br />As an initial reconnaissance of the mineshaft indicates that the mine will not need to be <br />dewatered during the project, water discharge is not planned. <br />6C.
