EPA - UIC - V- Application.doc 3/5/2009
4 of 6
? Chemical analysis of water from the receiving formation, overlying and
underlying aquifers (if already available).*
Please see attached "Water Analysis - Cash Mine - Mine Pool" datasheet compiled
from samples taken by MRV and analyzed by ACZ Laboratories, inc. in accordance
with the "Cash Mine Comprehensive Water Monitoring Plan"
? Description and chemical content of the proposed injectate; chemical analysis*, if
available.
The proposed injectate is a slurry of water and solids with a varying solid to water
ratio. The solid portion of the injectate is fine (100 mesh, or 149 g) rock excavated
from the void where proposed injection will occur. This material has undergone a
beneficiation process where approximately ninety-eight (98) percent of the contained
metals and sulfides are removed. Please see attached "Description and Chemical
Analysis of Proposed Injectate" by MRV.
? Description of hydrogeologic conditions at injection site, description, depth, and
current use (if any) of the aquifer formations on site; depth and direction of flow
of ground water.
Please see "Cash Mine Permit Revision Hydrogeology" by Adrian Brown, P.E.
attached.
? Location of existing monitoring wells (if any) and the location of any proposed
monitoring wells.
Please see "Cash Mine Comprehensive Water Monitoring Plan" - Plate 4 - by Adrian
Brown, P.E. attached.
? If injection is into an alluvial aquifer, provide locations of surface water bodies,
i.e. rivers, streams, and lakes, within one mile of injection site (may substitute
topographic map).
Injection is NOT into an alluvial aquifer. Injection will be into a low permeability,
minimally fractured, crystalline granite formation. One (1) ephemeral stream is
located at approximately 2500' from the point of injection.
? Provide location and description of any drinking water wells within one mile and
how they may be impacted by the proposed injection.
There are several drinking water wells within one mile of the injection site. Please
see "Cash Mine Borehole Non-Tributary Determination Support Information" - Plate
2 - by Adrian Brown, P.E. attached.
