Rule 6.4.4 Exhibit D- Mining Plan <br />5. Please specify whether the Operator plans to construct a slurry wall or a compact clay liner. <br />6. The Mining Plan states that the Applicant plans to install a perimeter drain for this operation. <br />Please provide all required design specifications, as well as a map of the location of this <br />drain, and the location of its planned discharge. <br />Rule 6.4.5 Exhibit E- Reclamation Plan <br />7. The exact location of the slurry wall/compact clay liner is unknown. Please provide a <br />structural analysis that verifies the minimum width or buffer of soil needed on either side of <br />the slurry wall/compact clay liner to provide adequate support during saturated soil <br />conditions, as well as under a rapid drawdown scenario. <br />The Operator must address the following factors in the reclamation plan: the design of the <br />slurry wall/compact clay liner (including construction plans and appropriate drawings), <br />technical specifications, construction quality control/construction quality assurance (CQA) <br />and commit to provision of a final construction report including the CQA Engineer's <br />certification. <br />9. The Applicant shall provide a plan or proposal of construction specifications that provides <br />specific measures to be implemented during varying site geologic conditions to assure that <br />the slurry wall/compact clay liner will be adequately keyed into competent bedrock <br />throughout the entire perimeter of the proposed installation, and minimize the potential for <br />excessive seepage losses once construction of the slurry wall/compact clay liner is <br />completed. <br />Rule 6.4.6 Exhibit F- Reclamation Plan Map <br />10. The submitted maps illustrate the approximate location of the proposed slurry <br />wall/compacted clay liner. The plans, as presented, are unacceptable for approval. The plans <br />will need to be revised to show the exact location of the slurry wall/compacted clay liner, or <br />the Applicant will need to commit in writing (and specify on the revised maps) the minimum <br />setback distance that the slurry wall/compacted clay liner will be installed from the permit <br />boundary. It is also necessary for the Applicant to clearly define in writing (and specify on <br />the revised maps) the minimum setback distance the slurry wall/compacted clay liner will be <br />from all surrounding permanent man-made structures. <br />11. Please provide a map, in sufficient size and scale, for all pertinent engineering and <br />water/hydraulic features. <br />Rule 6.4.7 Exhibit G- Water Information <br />12. The Division received comments from the Office of the State Engineer (OSE) regarding the <br />proposed application. According to the OSE, before a well permit can be approved the <br />Applicant must conduct a field location of the site and document the locations of all wells <br />within 600 feet of the permit area. The Applicant must then obtain a waiver of objection <br />from all well owners with wells within 600 feet of the permit area or request a hearing before
