Dear Ms. Ortiz,
Listed below are the additional items and information we request in order to complete our adequacy
review on the Henderson Development Amendment to a 112 Application, Permit No. M-1980-110. The
Division of Reclamation, Mining and Safety (Division) is required to make an approval or denial
decision no later than January 30, 2008, therefore, your response to the following adequacy review
concerns should be submitted to the Division as soon as possible.
As required by Rule 1.6.2 (1)(g), please submit the proof of publication of the public notice
circulated in the local newspaper, and proof of all required notices to all owners of record of
surface and mineral lands, easement holders, and owners of record of surface lands that are
within 200 feet of the boundary of the affected land. Proof of notice may be return receipts
of a Certified Mailing or by proof of personal service.
2. Please submit the following information for all permanent structures within 200 feet of the
affected area, including all utilities, well, and easement holders of record:
a) List of owners names and addresses
3. Exhibits B, G, H, I, J, K, M, O and P were omitted, and no statement referring to them was
provided with this current proposed amendment package. Please provide a statement
explaining which of the above required Exhibits remain unchanged from previous, approved,
amendments.
Rule 6.4.3 Exhibit C- Pre-Mining and Mining Plan Map(s) of Affected Lands
4. In conformance with Rules 6.4.3(b) & (f) and 6.4.7(2)(a), please provide a Water Resources
Map that shows the location of all streams, registered wells, springs, seeps, irrigation canals,
drainage channels, wetlands, or woody riparian vegetation within the existing permit area and
surrounding area.
