The <br />Division of Reclamation, Mining and Safety (Division) is required to make an approval or denial <br />decision no later than January 30, 2008, therefore, your response to the following adequacy review <br />concerns should be submitted to the Division as soon as possible. <br />As required by Rule 1.6.2 (1)(g), please submit the proof of publication of the public notice <br />circulated in the local newspaper, and proof of all required notices to all owners of record of <br />surface and mineral lands, easement holders, and owners of record of surface lands that are <br />within 200 feet of the boundary of the affected land. Proof of notice may be return receipts <br />of a Certified Mailing or by proof of personal service. <br />2. Please submit the following information for all permanent structures within 200 feet of the <br />affected area, including all utilities, well, and easement holders of record: <br />a) List of owners names and addresses <br />3. Exhibits B, G, H, I, J, K, M, O and P were omitted, and no statement referring to them was <br />provided with this current proposed amendment package. Please provide a statement <br />explaining which of the above required Exhibits remain unchanged from previous, approved, <br />amendments. <br />Rule 6.4.3 Exhibit C- Pre-Mining and Mining Plan Map(s) of Affected Lands <br />4.
