In conformance with Rule 3.1.6(1), please provide a map exhibit which shows the location <br />and describes the ownership of each Office of the State Engineer-registered well within six <br />hundred (600) feet of the proposed slurry wall/compact clay liner installation. <br />14. Please provide the Division with the current, approved Substitute Water Supply Plan on file <br />with the Office of the State Engineer. <br />15. Please submit information that clearly defines the gaining and/or losing nature of the Platte <br />River in the vicinity of the permit area; this information must be prepared by a professional <br />hydrologist or other qualified professional. <br />16. In conformance with Rule 6.4.7(1), please state affirmatively whether or not the proposed <br />slurry wall/compact clay liner installation is expected to directly affect surface or <br />groundwater systems within the affected land and surrounding area. If the proposed slurry <br />wall/compact clay liner installation is expected to directly affect surface or groundwater <br />systems within the affected land and surrounding area, please describe those anticipated <br />impacts in detail. <br />17. In conformance with Rule 6.4.7(2)(b), please provide a characterization of the alluvial <br />aquifer in the affected land and surrounding area which includes the following: <br />• Hydraulic gradient, and direction of groundwater movement <br />• Description of the Platte River for the reach proximate to the slurry wall/compact clay <br />liner installation (gaining or losing stream, channel stability, bank stability, presence of <br />riparian vegetation) <br />• Description of the normal surface/groundwater interaction and function throughout a 12- <br />month cycle in the affected land and surrounding area (including Platte River) <br />18. The possibility of groundwater fluctuations should be considered when developing design <br />and construction plans for the slurry wall/compact clay liner installation, and that fluctuations <br />in groundwater levels can best be determined by implementation of a groundwater <br />monitoring plan. Please state affirmatively whether such a groundwater monitoring plan will <br />be developed and implemented as part of the slurry wall/compact clay liner installation <br />project. If yes, please provide the details of the groundwater monitoring plan, such as <br />location of monitoring stations, frequency of monitoring, design of monitoring stations, <br />length of proposed monitoring, and purpose of groundwater data collected. <br />19. In conformance with Rule 6.4.7(2)(b), please provide a representative cross sectional <br />diagram for the mined lake which adequately depicts the features of the alluvial aquifer <br />within the affected land and surrounding area. The cross sectional diagram should be drawn <br />to scale and provide the following information: <br />• Existing reclaimed excavation(s) and lake high water mark(s) <br />• Groundwater (peizometric) surface <br />• Proposed slurry wall/compact clay liner installation <br />• 100-Year Floodplain boundaries (existing & proposed) & elevations <br />• Platte River - Floodway boundaries <br />3
