The Operator should <br />note that these structures must be placed within the permit boundary. <br />6.3.4 EXHIBIT D - Reclamation Plan <br />9. The Reclamation Plan calls for replacing the overburden once mining is complete. <br />This contradicts the Mining Plan which states that no overburden will be removed or <br />stored. Please clarify if the overburden will be replaced during reclamation. <br />10. The Reclamation Plan states that the stored topsoil will be replaced during <br />reclamation. Please clarify the amount of topsoil that will be replaced. The Mining <br />Plan indicates that 4 inches of topsoil will be removed prior to mining. The Division <br />recommends replacing 6" - 12" of topsoil. Please commit to replacing topsoil to a <br />depth of 6" - 12" or specify the minimum depth or range of depth for the reclaimed <br />areas of the site. <br />11. The seed mix that was submitted as part of the Reclamation Plan calls for applying <br />Little Blue Stem at 2% Pls #/Acre. Please clarify if this is a typographic error or if <br />Little Blue Stem will make up 2% of the seed mix. <br />12. The Reclamation Plan calls for applying 4,000 lbs of mulch per acre. Please specify <br />the type of mulch to be used. <br />13. The Division calculates the cost to reclaim the site based on the point of maximum <br />disturbance. Please determine and specify the point during the operation when the site <br />has reached a point of maximum disturbance. <br />14. The Reclamation Plan states that the pit slopes will be restored to 3H:1V. Please <br />provide the volumes of material that must be moved to create the slopes. In addition, <br />provide the haul or push distances for this material as well as the unit costs (cost per <br />cubic yard). <br />15. Please provide the costs for finish grading and topsoil replacement as cost per acre. <br />6.3.5 EXHIBIT E - Map <br />16. Rule 6.2.1(2)(b) requires that all maps, except the index map, be signed by a <br />registered land surveyor, professional engineer, or other qualified person. Please <br />submit signed copies of the Mining Plan Map and Reclamation Plan Map. <br />17. Rule 6.2.1(2)(e) requires that all maps, except the index map, include an appropriate <br />legend. Please include a legend on the revised Mining Plan Map and Reclamation <br />Plan Map. <br />18. Rule 6.3.5(2)(b) requires that the Mining Plan Map include the names of owner(s) of <br />record of the surface of the affected area and of the land within two hundred feet of
