the affected area. In addition, the owner of the substance to be mined must be <br />identified. Please include this information on the revised Mining Plan Map. <br />19. Rule 6.3.5(2)(c) requires that the Mining Plan Map include outlines and labels of all <br />major surface features to be used in connection with the proposed operation. Please <br />label the permit boundaries on the revised Mining Plan Map. Also, outline and label <br />the berm and swale ditch (see comments under No. 15, Exhibit C). The Mining Plan <br />Map only shows the location of the overburden stockpile, please outline and label the <br />topsoil stockpile (see comments under No. 5, Exhibit C). Finally, please label the <br />intermittent streams that are on either side of the affected areas. <br />20. Rule 6.3.5(2)(d) requires that the Mining Plan Map indicate the direction that <br />construction material extraction will proceed. Please include this information on the <br />revised Mining Plan Map. <br />21. Rule 6.3.5(3)(d)-(e)requires that the Reclamation Plan Map state the average <br />thickness of replaced overburden as well as the average thickness of replaced topsoil <br />by reclamation area. Please include this information on the revised Reclamation Plan <br />Map. <br />6.3.6 EXHIBIT F - List of Other Permits and Licenses Required <br />No comment. <br />6.3.7 EXHIBIT G - Source of Legal Right-to-Enter <br />No comment. <br />6.3.8 EXHIBIT H - Municipalities Within a Two-Mile Radius <br />No comment. <br />6.3.9 EXHIBIT I - Proof of Filing With the County Clerk <br />No comment. <br />6.3.10 EXHIBIT J - Proof of Mailing of Notices to Board of County Commissioners <br />and Soil Conservation District <br />22. Please provide proof that the permit application was sent to the Board of County <br />Commissioners and the local Soil Conservation District. Proof of notice shall be in <br />the form of a return receipt of a Certified mailing or a date-stamped copy of the notice <br />acknowledging receipt by the appropriate local Board.
