3. The test pressure during an MIT performed to demonstrate non-
endangerment during the temporary suspension period shall be no less than
300 psi. During the 30 minute pressure test, an observed pressure change of
more than 10% shall be deemed a MIT failure. The MIT shall be performed
by applying pressure to the casing above the bridge plug set in the well during
temporary suspension. At the conclusion of a test, the applied pressure shall
be bled to zero.
4. If a well does not successfully pass an MIT or non-endangerment
demonstration, the permittee may repair the well by submitting an
approvable corrective action (repair) plan to the Director within 30 days of
the MIT failure. Upon completion of the repairs, the permittee shall retest the
well and demonstrate mechanical integrity.
5. If a well does not successfully pass an MIT or non-endangerment
demonstration and the corrective action (repair) plan has not been submitted
to EPA for approval, it shall be plugged and abandoned in accordance with
an EPA-approved plugging and abandonment plan within 60 days, unless the
Director approves of an alternative method to demonstrate non-
endangerment and the well can meet those non-endangerment criteria.
6. Volumetric flow meters shall be installed on the well injection flow line or, for
multiple wells that are to be temporarily suspended, at a point on the injection
manifold flow line that is representative of the common operation of the
suspended wells. All meters shall be recorded twice monthly in accordance
with 40 CFR 146.33:
7. Pressure recording devices shall be installed on the well injection flow line or,
for multiple wells that are to be temporarily suspended, at a point on the
injection manifold flow line that is representative of the common operation of
the suspended wells. All meters shall be recorded twice monthly in
accordance with 40 CFR 146.33.
8. Monitoring of water quality and water level: The permittee shall carry out
the applicable monitoring requirements identified in the Surface Water
Monitoring Plan (Appendix I). The permittee shall utilize the applicable
analytical methods described in Table 1 of 0.40 CFR 136.3, or in Appendix III
of 40 CFR 261, and the QA/QC methods approved in Appendix I.
b. Monitoring Information. Records of any monitoring activity required under this
permit shall include:
1. The dates, exact place, and the time interval of sampling, monitoring, or field
measurements;
2. The name of the individual(s) who performed the sampling or measurements;
3. The exact sampling method(s) used to take samples; The date(s) laboratory
analyses were performed;
4. The name of the individual(s) who performed the analyses;
5. The analytical techniques or methods used by laboratory personnel; and
6. The results of such analyses.
FINAL Addendum to SOB For Modification No. 9, UIC Area Permit No. C030858-00000 Page 12 of 29
