c. Record Keeping. The Permittee shall retain records concerning: <br />1. All monitoring information and copies of all reports required by this <br />Temporary Suspension Plan for a period of at least, five (5) years from the <br />date of the sample, measurement, or report. <br />2. The permittee shall continue to retain such records after the retention period <br />specified in the above item unless he delivers the records to the Director or <br />obtains written approval from the Director to discard the records. <br />3. The permittee shall maintain copies (or originals) of all pertinent records <br />[Part II Section G (3)(a)] available for inspection at the office of: <br />American Soda, <br />2717 County Road 215 <br />Parachute, Colorado 81635 <br />d. Reporting of Results. The permittee shall submit Quarterly Reports to the Director <br />summarizing the results of the monitoring required by Part II Section G (3)(a). The <br />permittee shall report mechanical integrity test/ non-endangerment demonstration <br />results required by Section G (3)(a) in the quarter following the date of <br />demonstration. <br />The first Quarterly Report shall cover the period from the effective date of the <br />Suspension Notification through the end of that quarter. Subsequent Quarterly <br />Reports shall cover the periods of: January 1 through March 31; April l through <br />June 30; July 1 through September 30; and, October 1 through December 31. Each <br />Quarterly Report shall be submitted to the EPA Denver Office thirty (30) days after <br />the end of the quarter to the address below. Temporary Plugging: The borehole and casing of wells placed into temporary suspension <br />status must be securely plugged to prevent any movement of the fluid into or between <br />USDWs. <br />a. Temporary Plugging Plan. All wells to be placed in temporary suspension status are <br />deemed to be candidates for future operation. Any wells that are not deemed to be <br />candidates for future operations shall be plugged and abandoned using agency <br />approved methods in accordance with Part II Section E of this permit. The <br />permittee must temporarily plug suspended wells according to the following plan. <br />1.
