(MIT) is required at least every two years for as long as the well is <br />in temporary suspension status. <br />Results of any MIT(s) shall be submitted in the first quarterly <br />monitoring report after the MIT has been conducted and shall <br />include the following information: <br />o date, time, and interval of each MIT measurement; <br />o individual(s) who performed the MIT measurements; <br />o dates of calibration of the MIT equipment; <br />o techniques and equipment used to collect MIT <br />measurements. <br />The test pressure during an MIT performed to demonstrate <br />non-endangerment during the temporary suspension period shall be <br />no less than 300 psi. During the 30 minute pressure test, an <br />observed pressure change of more than 10% shall be deemed a <br />MIT failure. The MIT shall be performed by pressurizing the <br />casing above the bridge plug set in the well during temporary <br />suspension. At the conclusion of any such test, the applied <br />pressure shall be bled to zero. <br />If a well does not successfully pass an MIT or non-endangerment <br />demonstration and the permittee desires to repair the well, it may <br />do so by submitting an approvable corrective action plan to the <br />Director within 30 days of the MIT failure. Upon completion of <br />the repairs, the permittee shall retest the well and demonstrate <br />mechanical integrity. <br />If a well cannot successfully pass anon-endangerment <br />demonstration (i.e. the above-referenced MIT during the temporary <br />suspension period) and a corrective action plan has not been <br />submitted to EPA for approval, it shall be plugged and abandoned <br />in accordance with anEPA-approved plugging and abandonment <br />plan within 60 days, unless the Director approves of an alternative <br />method to demonstrate non-endangerment and the well can meet <br />those non-endangerment criteria. <br />+ Volumetric flow meters shall be installed on the well injection <br />flow line or, for multiple wells that are to be temporarily <br />suspended, at a point on the injection manifold flow line that is <br />representative of the common operation of the suspended wells. <br />All meters shall be recorded twice monthly in accordance with 40 <br />CFR 146.33. <br />FINAL: Modification No. 9, UIC Area Permit No. CO30858-00000 <br />Page 16 of 46 <br />
