Third Pany Oversight 2 Water, Waste & Land, Inc. <br />San Luis Mine January 10, 1993 <br />' • October 8 -BMRI suspends construction on the Raise I Embankment until review and <br />conclusion of outstanding issues raised by the DMG. <br />• October 28 -Site visit by DMG to initiate third-party review. <br />• November 17 -BMRI resumes construction of the Raise I Embankment upon approval <br />from DMG. <br />• December 9 -BMRI completes construction of the Raise I Embankment. <br />1.3 WORK SCOPE <br />' Third-party technical oversight was conducted according to the tasks outlined in the <br />October 15 Agreement. These tasks are outlined below. <br />' Task 1 -- Familiarization with Approved Plan. This work was started on October 25, and <br />included an independent review of pertinent design and permitting documents and correspondence <br />concerning the tailings disposal facility expansion. Documents provided by BMRI and DMG and <br />reviewed by WWL are listed in Section _ of this report. Key documents are cited in this report. <br />A meeting with DMG staff and BMRI staff was held at the mine site on October 28 to acquire <br />additional technical clarification on design and construction material issues. <br />Task 2 -- Field Construction Oversight. A construction oversight schedule was established <br />with BMRI and DMG. Field construction oversight was conducted on November 'i 1, 1993 and <br />from November 17 to December 8, 1993. Oversight activities included observatior~ of all phases <br />of expansion construction. Updates of construction activity were provided by WWL in weekly <br />reports to DMG. Immediate notification to DMG about construction activities of concern was to <br />' be provided; no activities of concern were observed during construction. Recommendations <br />regarding daily or seasonal suspension of construction due to weather conditions were reviewed. <br />Construction of the Raise I Embankment expansion was substantially completed on December 9, <br />1993. <br />1 <br />Task 3 -- Oversight of Field Testing Program. WWL reviewed the current and previous <br />(Phase p construction quality assurance (QA) testing programs, as well as construction QA test <br />results available from the initial stages of construction of the Raise I Embankment and expansion <br />of the tailings disposal facility. Opinions concerning the adequacy of testing and validity of the <br />' test results were provided to DMG with the weekly reports. As of the date of this report, several <br />confirmatory laboratory tests were still in progress. No tests were conducted by W/WL. <br />' Task 4 -- Review of Related Technical Submittals. WWL reviewed technical documents <br />prepared by BMRI as directed by DMG. WWL's technical opinions concerning these documents <br />are provided in this report. <br />' Task 5 --Review of Final Construction Certifications. If directed to do so, WUJL will review <br />the certification document from the Phase II, Raise I construction and will provipe DMG with <br />technical opinions concerning the document after it is completed by BMRI. <br />Task 6 -- Participation in Meetings with' BMRI. WWL participated in a technical review <br />meeting with DMG on November 2, and participated in informal telephone discussions with DMG <br />as needed.
