I October 2000 -16- 003-2191 <br />I <br />2) if the concentration of explosive gases in monitoring wells M 1 - M6 is over 5% <br />by volume in air for methane; and <br />3) immediately take all necessary steps to ensure protection of human health; and <br />4) within seven days of detection, place in the operating record documentation of <br />the explosive gas levels detected and a description of the actions taken; and <br />5) within sixty days of detection, implement an approved remediation plan, place <br />an approved copy of the plan in the operating record, and notify the Department <br />and local governing body that the plan has been implemented. The plan shall <br />describe the nature and extent of the problem and the proposed remedy. <br />b) if the gas detected is baseline or generated from the surrounding shale or <br />surrounding strata, the Department shall be notified and the Operator shall <br />propose a plan of containment or remediation. <br />2.4 RECORDKEEPING <br />A complete approved copy of the permit and engineering and operations plan shall be <br />maintained at the facility at all times and in the company's general office. The Operator <br />shall maintain all operating records at the faci]ity and the same shall be a part of the <br />operating record. The requirements of Section 2.4.2 shall be appropriately retained and <br />notification will be made to local authority and the Department. A sample of each <br />standazd form has been enclosed in Appendix 11. The standard forms shall include <br />inspection records, training and safety records, gas and water monitoring results, all <br />demonstrations, certifications, findings, and data, the closure and post-closure plans, <br />cost estimates and financial assurance demonstrations, and any applicable waivers and <br />exceptions to daily operating procedures. This information will be available to <br />appropriate local and state agencies at all times. <br />2.5 CLOSURE OF SOLID WASTE DISPOSAL SITES AND FACILITIES <br />The closure of this depository will conform with closure requirements of the <br />Regulations for MSWLF's. Not less than 60 days prior to anticipated closing, the <br />• Operator shall notify the Department in writing of the closing.
