<br />' A summary of test results identified as passing, failing, or, in the event of a failed test, <br />retest; <br />' • Test equipment calibrations, if applicable; <br />• A summary of decisions regarding acceptance of the work and/or corrective actions taken; <br />t and <br />• The signature of the Engineer's representative. <br />' 7.3 Construction Problem Report <br />' This report identifies and documents construction problems and resolutions. This report is intended <br />to document problems involving significant rework, and is not intended to document items which <br />' aze easily corrected unless the problems are recurring. At a minimum this report shall include: <br />• A detailed description of the problem; <br />' • The location and cause of the problem; <br />• How the problem was identified; <br />• How the problem was resolved; <br />• Personnel involved; and <br />' • Signature of the Engineer. <br />All construction problems will be reported by the Engineer's representative to both the Owner and <br />' the Engineer. In the event specific modification of construction activity cannot resolve the problem, <br />the problem will be reviewed for potential modification of the plans and specifications. <br />' 7.4 Photographs <br />' Construction activities shall be photographed, including significant problems and remedial actions. <br />The photographs shall be identified by location, time, date, and photographer. <br />7.5 Design and Specification Changes <br />' Design and Specification changes may be required during construction. If originating due to an <br />' identified construction problem, the Engineer or Owner shall suggest or recommend a design or <br />specification change. If a change in the plans or specifications is determined necessary, the Owner <br />will verbally notify the Division to determine if the change can be handled throuffih the final design <br />' repoR or if written notice and approval by the Division is required. Design and Specification <br />changes shall only be made with written agreement of the Engineer and Designer/Owner. <br />' e-z <br /> <br />
