the title of these documents to "Excess Spoil Fill Inspections" and include the date(s) of the <br />inspection(s) on the form. <br />1 ~` Quarter 2002 -Fill 7 <br />The Division did not receive a fill 7 first quarter inspection for 2002. Please submit this report. <br />April 12. 2002 -Fill 7 Specific inspection <br />Please change the title of the inspection form as noted on the fourth quarter comments above. <br />This inspection documents that the lower three terraces have been established at final grade of <br />approximately 3:1. No actual grade measurement was included. The report also documents <br />ongoing construction of the surface drainage system, specifically the armor channels or <br />downdrains. No specific information is provided other than a color photograph taken from a <br />distance was provided, Riprap sizing and channel requirements are detailed in Exhibit 13 of the <br />permit. <br />1 ~` Quarter 2002 -Fill 9 <br />The report is dated February 23, 2002 and indicates the outslope of the fill is being compacted <br />and final graded. The report indicates that completed grade would be verified by survey prior to <br />topsoil placement. The Division is aware that topsoil has been replaced prior to the May 2002 <br />inspection. Please include final survey information in the 2nd quarter report. The report <br />indicates the underdrain was complete at the time of the inspection although no photographs <br />were provided to verify underdrain construction. The photograph provided indicates one <br />uniform slope and is titled spreading and compacting fill. The CTL Thompson report in Exhibit <br />13 of the approved permit specifically states "where spoil placement is performed by dozing <br />down to the fill, care should be used to place fill in near horizontal layers and not create fill <br />layers which slope down toward the face. This could result in adversely oriented layers of finer <br />spoil which could affect stability" {page 17, April 27, 2001 CTL/Thompson report). Please ensure <br />that material is compacted in horizontal layers. <br />January 25, 2002 -Fill 8 Specific Inspection <br />This is the first inspection conducted for fill 8. The report cites construction of the underdrain <br />and installation of filter fabric. The report indicates photographs are attached but the Division <br />did not receive any photographs. Please submit photographs of the underdrain construction. <br />Under the report heading "Drainage System" it states "Fill area #8 is not a valley fill". Fill area <br />#8 is a valley fill as defined by DMG Regulations and Exhibit 13 of the approved permit, <br />Therefore, fill 8 must meet the requirements of Rule 4.09.1 and the additional requirements of <br />Rule 4.09.2. Please also change the title of the inspection form as noted on the fourth quarter <br />comments above. <br />1$' Quarter 2002 -Fill 8 <br />The report is dated March 30, 2002. This inspection meets the minimum requirement for one <br />inspection per calendar quarter. The report cites ongoing fill placement. The report states that <br />the underdrain construction is complete. No color photographs of the underdrain were found <br />with the report. Surface drainage has not been established and no benches have been <br />constructed. For the first time, the report includes a section titled "Recommended Actions", <br />citing the need for topsoil stockpiles to be shaped and seeded. <br />
