Long <br />Division Oiredor <br />The Division of Minerals and Geology has completed review of the 4'" quarter 2001 and 1st <br />quarter 2002 excess spoil fill inspections for Fills 7, 8 and 9 at the Lorencito Canyon Mine. <br />Please respond to questions or requests for information within 30 days of receipt of this letter. <br />Excess spoil fills 7 and 9 must meet the requirements of Rule 4.09.1 and the design standards <br />approved in Exhibit 13 of the permit application. Excess spoil fill 8 is a valley fill and must meet <br />the requirements of Rules 4.09.1 and 4.09.2, in addition to design requirements of Exhibit 13. <br />For all future reports submitted please add the following information; <br />• Please add to the certification the statement that Ronald Thompson, a qualified <br />professional specialist experienced in the construction of earth and rockfill <br />embankments, accomplished the inspection under the direction of Charles <br />McGlothlin. <br />• Please add to each report the date construction began on the fill <br />Please note on the form that the inspection report is due within two weeks of the <br />inspection (for those inspections done to comply with Rule 4,09.1(11)(b)). <br />• Please add to the report a section that addresses any instability, structural <br />weakness, or other hazardous conditions possibly noted during the inspection. <br />4th Quarter 2001- Fill 7 <br />Fill 7 construction also began in the 4't' quarter 2001 as the mining progressed to the west. The <br />report indicates photographs are included of the fill construction. The Division did not receive <br />the photographs. Please submit these photographs as soon as possible. Please also change <br />the title of these documents to "Excess Spoil Fill Inspections" and include the date(s) of the <br />inspection(s) on the form. <br />4'h Quarter 2001- Fill 9 <br />Fill 9 was the first fill constructed at the mine. Construction began in late October 2001. The <br />report indicates photographs are included of the fill construction. The Division did not receive <br />the photographs. Please submit these photographs as soon as possible. Please also change <br />
