July 17 - 18, 2002 Inspection by Steve Shuey: <br />Records: Review of Division records for the Blackburn mine, Anna May mine, Wilson <br />mine, Bull Durham/Last Chance vent, and the Bull Durham portal identified these as sites <br />which had been affected under the Permit and which are the reclamation responsibility of <br />the current Operator. A report on the Bull Durham portal and the Anna May vent (i.e. <br />Last Chance/Bull Durham vent) was requested from the Operator. <br />Response: On October 8, 2002, the Operator responded to acknowledge the <br />inclusion of the sites identified above, with the exception of the Bull Durham portal. The <br />Bull Durham portal was covered under Excel Mining Co.'s Last Chance Permit M-77- <br />468. On November 8, 2002 the Division confirmed that the Last Chance/Bull Durham <br />mine site is not included under the Operator's Permit. However, the vent hole is covered <br />under the Operator's Permit, and the Operator acknowledges that it has responsibility to <br />reclaim the vent collar and surface site. <br />Sims and Markers: An identification sign is needed at the Anna May mine while it is in <br />reclamation. The sign already located at the Blackburn mine is acceptable, and the <br />Wilson mine does not need a sign as this site is releasable from further reclamation. <br />Response: An identification sign was placed at the Anna May mine, and <br />documentation was provided to the Division on November 6, 2002. On November 8 the <br />Division acknowledge the Operator's actions and this Corrective Action item from prior <br />inspections was deemed resolved. <br />Blackburn reclamation tasks: Specific follow up final reclamation tasks were identified. <br />The lack of timely completion of reclamation was noted as a problem, and a Corrective <br />Action to provide a reclamation schedule by October 11, 2002 was stipulated. <br />Response: The Division was notified of the Operator's reclamation schedule on <br />October 8, 2002. <br />Anna May Reclamation tasks: Debris removal, decline trench closure, use of dump rock <br />for fill, and the utilization of local borrow materials for topsoil were noted. In addition, <br />the disposition of an associated vent shaft (unidentified) needs to be addressed. <br />Response: The noted reclamation tasks have been completed and reported upon <br />in the accompanying Site Description Report. A specific vent shaft associated with this <br />Permit and the Anna May mine cannot be identified from existing records. As shown on <br />the "Anna May & Wilson Mines" map, 1"=100', several vent shafts in the vicinity of the <br />Anna May mine were backfilled and permanently reclaimed by the Operator in October <br />2002. The Operator believes that either Vent Hole 1 or/and Vent Hole 2, as depicted on <br />the 1"=100' map, are associated with the Anna May mine; both vent shafts were <br />backfilled and reclaimed in October 2002. <br />2 <br />
