REVIEW OF PAST INSPECTION REPORT ITEMS <br />MONOGRAM - JO DANDY PERMIT M-1977-293 <br />MARCH 2003 <br />This Report provides a review of past Inspection Reports received from the Division <br />under Permit M-1977-293. The intent of this review is to document that all past inquiries <br />have been addressed by the Operator. <br />June 4, 2001 Inspection by Bob Oswald: <br />Corrective Action for failure to provide undated site information: The Operator had not <br />provided updated site status and condition reports for many years. <br />Response: The accompanying information submitted herewith addresses this <br />outstanding issue. Reports submitted in the past two years by the Operator have partially <br />addressed this issue, but uncertainty remained as there was not a consensus on which <br />sites were included under the Permit, which sites had been affected after Permit issuance, <br />and which sites are the responsibility of the Operator. All of these issues have now been <br />resolved. <br />Corrective Action for lack of identification signs and boundary markers: No Permit <br />identification signs were present and boundaries of affected areas are not marked. <br />Response: The Operator placed Permit identification signs in November 2001. <br />This response action was complicated by the number of sites and their scattered locations. <br />A general Permit identification sign was located at the Blackburn Mine and, in October <br />2002, an individual mine identification sign was located at the Anna May mine. All sites <br />which have been confirmed to be covered under Permit M-1977-293, and which are <br />bonded for final reclamation by the Operator, are in final reclamation. The Operator <br />understands that boundary markers are not required for sites under final reclamation. <br />November 27, 2001 Inspection by Steve Shuey: <br />Corrective Action continuation for identification sins and markers: A follow up <br />verification inspection was conducted to determine compliance with outstanding <br />Corrective Actions items. Partial compliance by the Operator was noted by the Division, <br />but resolution of the outstanding Corrective Actions was deferred until a joint site <br />inspection could be performed with the Operator during fair weather conditions. <br />Response: The proposed joint inspection was held in July 2002. The Division <br />and the Operator followed this inspection by carefully examining existing file records and <br />determined which sites have been affected under the Permit and require follow up <br />reclamation by the Operator.
