the Permit site is a few hundred feet due west of the historical disturbed area shown in the <br />lower right comer of the Blackburn Mine 1"=100' map; a portal is labeled "Blackburn <br />Portal" on this map as a number of sites may have actually been referred to as <br />"Blackburn". The omission of the correct affected site will be rectified this summer, as <br />additional surveying work will be conducted; an updated site map will be submitted with <br />the Annual Report to the Division in November. <br />All of the noted reclamation and maintenance activities were completed by the Operator <br />in October 2002. In addition, several Operator Diligence activities were completed to <br />reduce potential hazards due to open historical portals and unstable highwalls. During <br />the Operator Diligence work, access into the affected sites was improved to allow <br />equipment access and to fix drainage and runoff problems. All areas that were entered <br />and impacted by reclamation equipment were scarified and seed was hand broadcast <br />following site activities. The following seed mix was used in all affected areas: <br />Crested Wheatgrass: 5 - 7 lbs/ac. <br />Western Wheatgrass: 2-3 lbs/ac. <br />Four Wing Saltbush: 3.3 - 4.4 Ibs/ac. <br />Indian Rice Grass: 3.3 - 4.4 lbs/ac. <br />Sand Drop Seed: 0.3 Ibs/ac. <br />Yellow Sweet Clover: 1 ]b/ac. <br />Total Seed: I S - 20 Ibs/ac. <br />This seed mix was obtained from the existing reclamation plan for the Permit and the <br />Monogram Mines Permit. <br />During the July 2002 inspection, an inquiry was made by the Division as to the <br />ownership of the overhead power line into the Blackburn area. The Operator has <br />contacted the local utility, and the utility has agreed to review the status of power <br />installations and the related ownership thereof. This update will be performed at several <br />of the Operator's Permit sites throughout the region. Upon receiving the update from the <br />utility, this information will be forwarded to the Division. <br />Anna Mav Mine• <br />The Anna May Mine consists of an open decline and nearby waste dumps. This mine is <br />located in the midst of a number of historical mine sites and a mine site which is covered <br />under the Monogram Mines Permit M-1978-222 UG, see the Anna May & Wilson Mines <br />M-1977-293 map, 1"=100'. <br />The Anna May & Wilson Mines map shows four types of sites: <br />- "Historical Mine Sites." These sites are shown on the map for reference. The <br />Operator elected to locate and survey historical sites that had been referenced <br />under original Permit applications, but which were never affected after Permit <br />issuance. This was done to facilitate future site visits and to establish a <br />permanent record with the Division. <br />- "Monogram - Jo Dandy Permit M-1977-293" sites. These sites include the <br />Anna May and Wilson mines. <br />2 <br />
