SITE DESCRIPTION REPORT <br />MONOGRAM - JO DANDY PERMIT M-1977-293 <br />MARCH 2003 <br />After extensive field examination and review of existing Division and Operator records <br />during 2002, resolution was reached on which mine sites are included under the Permit, <br />and which therefore carry obligations for final closure and reclamation by the Operator. <br />The following mine sites are included under the Permit and have been affected by past <br />activities after Permit issuance: <br />- Blackburn Mine <br />- Anna May Mine <br />- Wilson Mine <br />- Last Chance vent <br />The locations of these mine sites is shown on the PROJECT LOCATION MAP, 1"=300'. <br />Numerous historical mine sites exist throughout the Monogram Mesa area. The Operator <br />also has one site that is covered under the Monogram Mines Permit M-1978-222-UG. <br />This site is shown on the PROJECT LOCATION MAP also. In addition, there are a <br />number of mine sites shown on the PROJECT LOCATION MAPS that have closed by <br />the Operator as part of Operator Diligence efforts to reduce potential hazards. <br />Blackburn Mine: <br />The Blackburn Mine is located on steep hillside that has a myriad of historical roads, <br />mine sites, waste dumps, etc., see Blackbum Mine map, 1"=100'. Following a joint site <br />inspection with the Division on July 18, 2002, the Operator conducted final reclamation <br />tasks at the Blackburn Mine site and had the Blackbum site and surrounding historical <br />affected areas surveyed. Unfortunately, given the abundance of affected sites in this <br />vicinity, the surveyors mistakenly did not survey the Blackburn Mine site that is the <br />subject of Permit M-1977-293. <br />The Blackburn Mine site under the Permit consists of a previously closed portal, a <br />hillside waste dump, and a water tank pad above the portal. This site has been <br />permanently closed. During the site inspection, the following follow up final reclamation <br />items were noted by the inspector: <br />- trash needs to be removed in the area of the water tank storage pad; <br />- additional material needs to be placed against the closed portal as a small <br />opening has developed; <br />- the dump top surface needs to be reseeded and scarified to supplement <br />existing vegetation; and <br />- debris from an old collapsed building needs to be removed. <br />The Operator completed the maintenance and reclamation tasks identified, but the portal <br />and tank pad area is not shown on the enclosed map.
