13) Following the reporting procedure outlined above, detailed information <br />about the spill should be gathered by the Spill Witness and his or her <br />immediate supervisor. This information should be placed on the <br />SPILL/RELEASE FACT SHEET in Appendix D. The required information <br />includes but is not limited to: <br />a. Date of spill <br />b. Time of spill <br />c. Exact location of spill <br />d. Estimated quantity of spill <br />e. Names of persons involved in, or knowledgeable about the <br />circumstances surrounding the spill <br />f. Occurrence of personnel exposure to hazardous materials (i.e. <br />skin contact, eye contact, ingestion, etc.) <br />g. Whether the spill has been controlled by expedient berms, etc.; <br />or whether the spill cannot be readily controlled before it <br />discharges into a waterway. <br />h. An estimate of the potential for the spill entering a pond or <br />waterway and the time before that occurs. <br />(4) If a potentially reportable spill occurs on a~ shift, it must be reported <br />as soon as possible to the Environmental Site Coordinator, who will <br />contact the Environmental Affairs Manager and the Colorado <br />Department of Public Health & Environment, if necessary. <br />(5) Downstream water users within five miles of the mill facility are very <br />unlikely to be affected, because any spilled materials would report to <br />and be contained in the tailing deposition area. <br />SPILL RESPONSE EQUIPMENT and SUPPLIES <br />Materials for spill control and clean-up are located throughout the Hender:>on <br />mill site. Shovels, floor dry, and empty drums or 5 gallon containers are <br />located in most of the buildings onsite. Spill control kits (including sorbent <br />booms and pads) are located in the Hazardous Materials 1 Flammable <br />Materials Storage Building. Granular limestone surrounds the Hydrochloric <br />Acid Storage Area and soda ash is stored in the orange tank north of the rnill <br />building; both of these materials can be used to neutralize acid spills. <br />Similarly, hypochlorite (potable water treatment plant) and sodium hydroxide <br />(mill building! can be used to convert spilled cyanide to cyanate. Skid loaders <br />and front-end loaders are located in the Mobile Equipment Shop and various <br />other site locations. <br /> <br />Page 17 <br />
