The Discharge Moaitotittg Report forms shall be filled out <br />accurately and completely in acoordaace with nxluiremeats of this permit and the instructiooa on the forms. They shall <br />be signed by as authorized person ae identified in Part I.D. <br />3. Acute VVEI' Testine-0utfalls 004. 023. 024 and 026 <br />(a) Testine and Reoortine Requiremwts <br />Tests shall be done at the froquwcy listed in Part LB.1. Test readts chap be reported along with the Discharge <br />Monitoring Report (DMR) submitted for the reporting period during which the sample was tekom. (i.e., WET testing <br />results for the first calwdar quarter wdiag March 31 shall be reported with the DMR due April 28.) The results <br />shall be submitted oa the Acute Toxicity Test report form, available from the Division. Copies of these reports are <br />to be submitted to both the Division end EPA along wish the DMR. <br />The permittee shall conduct each acute WET test in gweral accordance with methods described in <br />90/027 or the most currwt edition, eztxpt as modified by the most current Division guidance documwt wtitled <br />Guidelines for l'onductine Whole Eflluwt Tonicity Tests. The petmittee shall coad[rct an acute 48-hour WET test <br />using Daphnia Maene sp., and as awls 96-hour WET test using fathead minnows. Acute tests will be replacement <br />static tests of a single effiuwt grab sample. <br />(b) Failure of Test and Division Notification <br />Aa acute WET test is failed whomever the LCD, which represwts an estimate of the effiuwt coacomtratioa which is <br />lethal to 50:6 of the test orgmisms is the time period prescnbed by the test, is found to be less than or equal to <br />10056 effluent. The pemtittee must provide writtw notification of the failure of a WET test to the Division, along <br />with a atatemomt as b whether the Preliminary Toxicity Incidomt ('PTI')/I'oxicity Idomtification Evaluation ('TIE') <br />investigation or acceleratod testing is being performed (see nett section). Nolitimtion must be received by the <br />Division within 14 calendar days of the demonstration oC acute WET in the routine required lest. <br />'Demonstration' for the purposes of Parts I.B.4(b),(c),(d) and (f) means no later thaw We lest day of the laboratory <br />test. <br />(c) Automatic Compliance Schedule Ump Failure of Test <br />if a routine acute WET test is failed, the following automatic compliance schedule shall apply. As part of this the <br />permittee shall either: <br />(i) proceed to conduct the PTI/fIE investigation as described in Part I.B.4.(d), or <br />(ii) conduct accelerated testing using the single species found to be more sensitive. <br />If accelerated testing is being performed, the permitlee shall provide written notification of the results within <br />14 calendar days of completion of the "Pattern of Toxicity"/"No Toxicity" demonstration. Testing will be at <br />least once every two weeks for up to five tests until 1) trop consecutive tests fail or three of five tests fail,=in which <br />case a pauem of tonicity has beom demonstrated or, 2) two consecutive tests pass or three of five tests pass, in <br />which case no pattern of toxicity has been found. <br />m'nnr ucnr <br />
