Gravel pack material will then be measured and placed in the annular space and <br />• allowed to settle to 37'. ARer the desired amount ofgravel pack has been placed <br />in the annular space, a cement slurry will be prepared, measured and placed in the <br />annulus of the well to within 3' of the surface. A 12" x .250" steel casing and <br />identification cap will then be place over the 6" PVC for protection and finished to <br />ground level. During the placement of both the gravel pack and grout materials, <br />the casing will be tapped to help eliminate air pockets and promote settling. <br />A Geologist will be on location to log the well and insure correct procedures are <br />followed. <br />Abandonment of Old AMW-2 <br />The sealing procedure will follow the Divisions Regulations 4.07.3 (Permanent <br />Sealing). Cement grout will be poured down the 4 1/2" casing annulus to within <br />three (3) feet of the ground surface. The casing will be cut offthree (3) feet below <br />the surface, capped, and back filled. During the placement of grout materials, the <br />casing will be tapped to help eliminate air pockets and promote settling. <br />Post Completion Data <br />Within sixty (60) days the Division of Minerals and Geology will be provided in <br />• writing with the following: <br />* Update pages for section 2.04.7 (Hydrology), relating to AMW-2 <br />* Current status of new AMW-2 well <br />* Well log and detailed procedures used <br />* Detailed description of old AMW-2 abandonment <br />* Updated well location and elevation map (AI-IR 1994) <br />* Updated Piezometric Surface of Overburden Aquifer drawing, with data from <br />existing wells.
