<br />6.4.13 Exhibit M Amendment Other Permits and Licenses <br />(revision 5/01) <br />Shook Gravel Pit #1 <br />Township 1 North, Range 52 West <br />SW 114 of Section 26 <br />S 1/2 of Section 27 <br />As stated previously in the exhibit submitted, to the best of our knowledge, <br />all of the necessary permits, licenses and approvals that we need have <br />been obtained in order to conduct the proposed mining and reclamation <br />operations above. <br />Each suggested permit /license & approval is listed below. <br />Applicant does not hold nor will be seeking the permits for the <br />following reasons: <br />Effluent discharge permit- Will not be impeding drainage of dry creek bed. <br />Air quality emissions permit- Mining wilt not effect air quality of the area. <br />Radioactive source material licenses- No known radioactive materials in the <br />area. <br />State Historic Preservation Office clearance- No historical sites in the area. <br />D__isposal of dredge & fill material (404~permit-Will not be disposing of <br />dredge or fill material. <br />Permit to construct adam- Will not be constructing a dam. <br />Well permits- Will not be drilling or using well water. Well permits enclosed. <br />Hiwav access permits- Mining is not near or entering a hiway . <br />U.S. Forest Service permit- Not within a U.S. Forest Land area. <br />Bureau of Land Management permit- Not on or near BLM land. <br />County zoning and land use permit- Applicant is the County. <br />City zoning ~ land use permit- Not within a city zoning area. <br />While inquiring on other possible permits, we were advised that at <br />this Time, permits were not applicable due to the county size and small <br />population. <br />The only permit Washington County Road & Bridge Department has <br />applied for is the 112 Construction Materials Reclamation Permit from the <br />Division of Minerals and Geology of Colorado. <br />Washington County Commissioner <br />
