The Colorado Registered Professional Engineer shat( seal dte.plans in accordance with <br />CRS 12-'_J-I 17 in addition on the tirst page of the design plans that [he following note is required: <br />"This design is in full compliance With Section 1 Of [he State Highway Access <br />Code. 2 CCR 601-I except for the following approved variances' <br />Note: ALL plans must be submitted on 11" x 17" sheets at a minimum scale of <br />1" = 20'. No other plan sheet sizes are authorized. Location drawings may <br />be submitted at other scales as approved. <br />24. All construction work must be under the direction of a Colorado Registered Professional Engineer and upon <br />completion of the work. [hat responsible Engineer shall submit an "As Built" plans, showing in detail all approved <br />construction chances. modifications and revisions. The "As Built' Plans must be sealed in accordance with CRS <br />12-25-117. <br />25. It shall be the responsibility of the Permittee to verify the location of the existing utilities and notify all utility <br />owners or operators of any work that migh[ involve utilities within the State Highway right-of-way. Any work <br />necessary to pro[ect existing permitted utilities, such az an encasement will be the responsibility of the Permittee. <br />Any damage or disruption to any utilities during the construction shall be the Permittee's responsibili[y and shall <br />be repaired or replaced at no cos[ to the Department. <br />26. Any damage to any present highway facilities including traffic control devices shall be repaired immedia[ely at no <br />cast to the Department and prior to con[inuing other work. Any mud or other material [racked or otherwise <br />deposi[ed on the roadway shall be removed daily or az ordered by the Department inspector. <br />27. Areas of roadway and/or right-of-way disturbed during [his installation shall be restored to their original • <br />conditions, to insure proper strength, drainage and erosion control. <br />28. Any incomplete construction activity on the Stale Highway that must be left overnight. shall be barricaded and <br />signed in accordance with the Manual on Uniform Traffic Control Devices and other applicable standards. <br />29. Open cuts, which are 6 inches in depth, within 30 feet of [he edge of the Slate Highway [raveled way, will no[ be <br />left open at nigh[, on weekends, or on holidays. <br />30. No more [hen 6 fee[ of trench areas shall be opened a[ any one time. Open trenches and other excavations within <br />the State Highway right-of-way shall be backfilled and/or paved before 3:30 P.M. of each working day or be <br />protected in accordance with the M.U.T.C.D. -' <br />31. The area around the new work shat( be well graded to drain, top soiled, fertilized, mulched and re-seeded in <br />accordance with the Depanment standard specifications. <br />32. When it is necessary to remove any highway right-of-way fence, the posts on either side of [he access entrance <br />shall be securely braced with approved end posts and in conformance with the Department's M-607-t s[andard. <br />before the fence is cut, to prevent slacking of the remaining fence. All posts and wire removed shall be returned to <br />the Department. - <br />33. All excavations for utility lines, culverts, trenches or tunnels shall meet the requirements of [he Occupational. <br />Safety and Health Adminisvation (OSHAj. Colorado Industrial Commission. Colorado Division of Mines or the <br />Colorado Department of Transportation, whichever applies. <br />34. Alf work that requires traffic convol shall be supervised by a registered professional traffic engineer or by a traffic <br />control supervisor certified by the American Traffic Safely Services Association (ATSSAj or the Colorado <br />Con[ractors Association (CCA). When flagging personnel are required. the convector in accordance wi[h [he <br />Departmen[ standards shall certify them. <br />35. Construction vaffic control devices, when not in use, shall be removed or [urned away from traffic. <br />36. All temporary pavement stripin¢ shall be done by the Permittee/contractor in conformance with section 627 of Tt~ <br />Department's standard specifications for Road and Bridge Construction (latest editions. <br />i- <br />
