STATE HIGHWAY ACCESS PERMIT #399057 June 7, 1999 <br />issued-EO Bowie Resources. Inc. <br />TERMS & CONDITIONS (cont'd) <br />37. The Permiuee shall do all [final sisnin~ and pavement striping or the contractor in conformance with the <br />Department's M 3 S standards and the bt.U.T.C.D.. unless otherwise agreed to by [he Department and [he <br />Permittee. A final signing and pavement striping plan shall be submitted to [he Region 3 Traffic Engineer a <br />minimum of 45 working days prior to commencement of work. No work shall begin without prior approval and <br />authorization from the Department. <br />38. The roadway shoulder shall be widened to 4 feet along [he speed change lanes and surfaced with HBP in <br />accordance with the Department specifications. The shoulder widenins along through lanes shall be no less than 4 <br />feet paved or match existin: and paved. <br />39. The access shall be constructed perpendicular to the travel lanes of the S[a[e Highway for a minimum distance of <br />50 feet, and shall slope down and away from the adjacent pavement edge at a rate of 2r7c grade for a minimum of <br />20 feet. [f curb and gu[ter are present, the slope shall be calculated from pan line to pan line. Any revisions [o this <br />requiremen[ shall be subjec[ to Departmen[ review and approval prior [o commencement of any work within [he <br />highway right-of-way. <br />40. The access shall be completed in an expeditious and safe manner and shall be finished within 45 days from <br />initiation of construction within Stale Highway right-of-way. <br />41. Pursuant to section 4.9(3) of the Stale Highway Access Code, the access roadway shall no[ exceed a maximum <br />grade of 8 percent within the highway right-of--way, az measured 50 feet beyond the pavement edge and extending <br />to the righ[-of-way line. The access vertical grade shalt be designed and consvuc[ed in conformance with the <br />Department M & S standard M-203-1. <br />42. The desien of the horizontal and vertical sight distance shall be no less than the minimum requirements. az <br />provided in section 4.3 of the State Hiehway Access Code. 2 CCR 601-1. <br />43. All required access improvements shall be installed prior to the herein-authorized use of this access. <br />44. The access shall be surfaced immediately upon completion of earthwork consvuccion and prior to use. <br />45. Compaction of Hot Bituminous Pavement shall be in accordance wi[h section 401.17 of the Department's standard <br />specifications. Compaction of the Aggregate Base Course shall comply with section 304.06. <br />46. Compaction of subgrade, embankments and backfill shall be in accordance to section 203.07 of the Department's <br />standard specifications. <br />47. Any mud or other material vacked or otherwise deposited upon the existing roadway surface shall be removed az <br />necessary to accommodate vaffic, or as directed by the department. <br />48. Placement of base course materials shall be in accordance with section 304.04 of the standard specifications. <br />Compaction shall be in conformance with AASHTO procedure T-99. <br />49. The surfacing shall meet thz Department's specifications az ou[lined Materials Recommendations obtained from <br />the Region 3 Materials Engineer. Contac[ the Access Manager at (970) 248-7231 to request a materials <br />recommendation. - <br />50. The new Stale Hiehway pavement shall slope on [he same plane az [he present pavement surface. <br />S 1. Slopes shall be at a 6:1 ratio on the roadway and a 6:1 ratio on the approach. <br />52. The existing asphal[ adjacent [o alt new pavement shall be saw cut and removed a minimum of one foot back from <br />the existing edge, or until an acceptable existing cross slope is achieved to azsure a svaight edge for the joint. <br />53. The [op layer of plant mix bituminous pavemem shall not be placed between October 1 and April 1, unless <br />otherwise approved by the Depar[men[. <br />54. If frost, water or moisture is present in the subgrade. no surfacin_o materials shall be placed until all frost, water or <br />moisture is cone or remo~~ad. <br />3• <br />
