17. It is the responsibility of the Permiuee to prevent all livestock from entering the Star Hiehway right-of-way at this
access location. Any lives[vck that does enter the highway rightwf-way shall be the solr~responsibiliry of the
Permiuee.

that it be removed promptly by the Permittre and at no cost to the Department.
19. Landscaping shall not obsvuct sigh[ distance at any Stale Highway access point.
20. A fully executed complete copy of this permit must be on the job site with the convector at all times during the
construction. Failure to comply with this or any other consvuction requirement may result in the immediate
suspension of work by order of [hz department inspector or the issuins authority.
2l. All materials, equipment, installation. consvuction and design, including the auxiliary lane(s) and intersection
improvement(s) within the State Highway shall be in accordance with the following Department standard
references, as applicable:
(a) Roadway Design Manual
(b) Materials Manual
(c) Construction Manual
(d) Standard Specifications for Road and Bridge Construction, latest edition
(e) Colorado Standard Plans (M & S Standazds)
(f) Manual on Uniform Traffic Convol Devices (M.U.T.C.D.) for Sweets and Highways and the
Colorado Supplement thereto

(g) A Policy on Geomevic Design of Highways and Sweets, American Association of S[a[e
Highway and Transportation Officials (AASHTO). latest edition

22. Survey markers or monuments found in state highway right-of-way must be preserved in their original positions.
Notify the Department at (970) 248-7"'20 immediately upon damage to or discovery of any such markers or
monuments at [he work site. Any survey markers or monuments disturbed during the ezecutiort of this permit shall
be repaired and/or replaced immediately at the expense of the Permittee.
23. The Permittee, through a Colorado registered professional engineer shall provide design, conswuc[ion. pavement
striping and signing plans to the Department for approval 45 days prior to commencement of any work for the
purposes of issuins a Notice [o Proceed. Design plans must include but not limited to layout of speed change
lanes, utility locations. present and proposed drainage. presem and proposed right-of-way lines. present and
