Phase IV Weekly Summary No. 40 Page 2 003-2203 <br />Ames continued excavation and removal of the old Highway 67 asphalt, fence, and concrete <br />crossing. A Cat 375 loaded several Cat 777 haul trucks, which then hauled the waste to the "Sky <br />Dump" <br />Ames resumed SF blast drilling in Phase IVa at EOL station 21+00 and 9640 road station I 1+00. <br />Ames blasted on November 9s' at 16:05. <br />Phase IVb <br />Ames did not perform excavation remediation of Phase Nb underground workings during this <br />reporting period. <br />Ames performed clearing and grubbing of topsoil and removed trees. A Cat D7 and a Cat 330 track- <br />hoe removed trees and stripped topsoil from tha southwestern azea of Phase IVb. Topsoil and slash <br />were piled locally for relocation at a later date. <br />Ames conducted blast drilling where confirmation drilling had identified underground workings <br />within the remediation zone at UG-206. A lateral at UG-206 was blasted on November 7a' at 17:11. <br />Ames conduced confirmation drilling on known underground workings UG-206 and UG-217. <br />Laterals were completely delineated. <br />Ames continued to excavate azound and demolish the retaining wall at the Carlton Mill. Ames used <br />• Caterpillaz 330 and 375 excavators to remove material and wall. <br />Phase IVc <br />Ames performed excavation remediation activities for underground working UG-234a. UG-234a <br />will be remediated as a "Deep Shaft". <br />Ames performed clearing and grubbing of topsoil and removed trees. A Cat D7 and a Cat 330 track- <br />hoe removed trees and stripped topsoil from the northwestern area of Phase IVc, neaz the existing <br />topsoil pile (Located west of the Phase IVc limits). Topsoil and slash were piled locally for <br />relocation at a later date. <br />Ames did not perform any cut or fill activities within the Phase IVc azeas during this reporting <br />period. <br />(See Attachment A for approximate construction locations for all Phase IV azeas.) <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase N earthworks. All construction <br />materials and soil materials were verified as suitable and construction methods were in accordance <br />with project specifications.
