Phase IV Weekly Summary No. 51 Page 3 <br />Ames performed excavation remediation activities of known underground workings UG-508, UNK-08, <br />and UNK-10. These workings were observed and verified to be a "Deep or Shallow Shaft" as defined <br />in the drawings and will be remediated accordingly. <br />• Ames continued placement of Coazse Shaft Backfill (CSB) into "Deep" underground working UNK-08. <br />Ames resumed placement of Concrete Plug (CP) concrete into "Deep Shaft" UNK-08. Ames placed <br />three feet of concrete in accordance with IFC Drawing 12, Detail 1. <br />(See Attachment A for approximate construction locations for all Phase IV azeas.) <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase IV earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. Removal of oversized and deleterious material from all fill <br />areas was observed. Golder conducted temperature measurements of fill material prior to placement. <br />Golder observed recontouring of the Phase III ore slope. Ore was regraded to approximately 4H: ] V <br />and at no time was allowed off of the existing liner system. <br />Golder observed construction of the Primary Underdrain and Secondary Underdrain in Phase IVa <br />and Phase fVb for trench dimensions (5' by 3' and 4' by 2'), slope (minimum 1%), geotextile (12 <br />oz.), UF, geotextile overlap (2'), and SSF. <br />Golder observed concrete placement at the 84-inch wingwall footing. Prior to concrete placement <br />Golder observed form dimension and reinforcement spacing. Golder observed concrete testing <br />performed by Ground Engineering for the proper slump (2"-5") and air entrainment (5%-8%}. <br />Compressive strength samples were taken by Ground Engineering to determine the 28-day <br />compressive strength of the concrete placed. <br />Golder observed placement of Soil Liner Fill at the LOB extension for subgrade prepazation, <br />materials placed, and moisture content. <br />Golder observed placement of CSB, SSF, and CP. These materials were placed in accordance with <br />the drawings and specifications into excavated or open underground workings. Golder previously <br />obtained samples of SSF, CP, and CR for laboratory testing. Materials supplied met specifications. <br />Ground Engineering conducted field testing and sampling of CP concrete as per specifications, and <br />will supply 28-day strength results. <br />Golder observed and documented excavation and remediation activities associated with the Phase IV <br />underground working construction. Full time monitoring was given to excavation of underground <br />workings. <br />Golder observed placement of geogrid at UG-494.
