Drawing 12, Detail 1. <br />Phase Nc <br />Ames performed confumatory drilling activities associated with the Phase IVc construction area on <br />known underground workings UG-294b, UG-498, UG-499, UG-500, UG-504, EI Line-1, EI Line-2b, <br />and EI Line-2c. <br />Ames performed excavation remediation activities of known underground workings UG-208, UG-221, <br />UG-508, UG-511, and UG-512. These workings were observed and verified to be a "Deep or Shallow <br />Shaft" as defined in the drawings and will be remediated accordingly. <br />Ames continued placement of Coarse Shaft Backfill (CSB) into "Deep" underground workings UG-270 <br />and UG-498. <br />Ames continued blast drilling of underground workings within Phase IVc at UG-297, UG-257, UG- <br />499, and UG-500. Ames blasted underground workings delineated by confirmation drilling at UG-297, <br />UG-257, UG-499, and UG-500 on March 25d', March 26"', April 2nd, and April 4d' respectively at 16:30. <br />Ames resumed placement of Concrete Plug (CP) concrete into "Deep Shaft" UG-301. Ames placed <br />three feet of concrete in accordance with IFC Drawing 12, Detail 1. <br />Ames placed Cemented Rock&11 (CR) at UG-267, UG-268, UG-278, and UNK-09 by tapering the <br />working, and placing 7 feet of CR in accordance with IFC Drawing 12, Detail 1. <br />(See Attachment A for approximate construction locations for all Phase IV areas.) <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase IV earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. Removal of oversized and deleterious material from all fill <br />areas was observed. Golder conducted temperature measurements of fill material prior to placement. <br />No fill was placed until fill temperatures met the requirements of the specifications. <br />Golder observed construction of the Primary Underdrain and Secondary Underdrain in Phase IVa for <br />trench dimensions (5' by 3' and 4' by 2'), slope (minimum 1%), geotextile (12 oz.), UF, geotextile <br />overlap (2'), and SSF. <br />Golder observed concrete placement at the 84-inch headwall. Prior to concrete placement Golder <br />observed form dimension and reinforcement spacing. Golder observed concrete testing performed <br />by Ground Engineering for the proper slump (2"-5") and air entrainment (5%-8%). Compressive <br />strength samples were taken by Ground Engineering to determine the 28-day compressive strength <br />of the concrete placed. <br />Golder observed placement of CSB, SSF, CP, and CR.
