Phase IV Weekly Summary No. 47 Page 3 <br />003-2203 <br />267b, UG-267c, UG-278, UG-287, and UG-508. The workings were observed and verified to be <br />"Shallow" or "Deep" as defined in the drawings and will be remediated accordingly. <br />Ames performed confirmatory drilling activities associated with the Phase Nc construction area on <br />known underground workings UG-267, UG-268, and UNK-1. <br />Ames resumed placement of Concrete Plug (CP) concrete into "Deep Shafts" UG-254, UG-256, UG- <br />258, UG-260, UG-262, and UG-282. Ames placed three feet of concrete in accordance with IFC <br />Drawing 12, detail 1. <br />Ames continued blast drilling of underground workings within Phase Nc at UG-268. Ames blasted <br />underground workings delineated by confirmation drilling at UG-268 on February 13a' at 16:50. <br />(See Attachment A for approximate construction locations for all Phase IV areas.) <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase N earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. Removal of oversized and deleterious material from all fill <br />azeas was observed. Golder conducted temperature measurements of fill material prior to placement. <br />No fill was placed until fill temperatures met the requirements of the specifications. <br />Golder observed cleazing and grubbing within the Phase IV azea. All available topsoil was removed <br />. and deleterious material hauled to Upper Arequa Gulch. <br />Golder observed placement of SF, SSF, and CP. These materials were placed in accordance with the <br />drawings and specifications into excavated or open underground workings. Golder previously <br />obtained samples of SF, SSF, and CP for laboratory testing. Materials supplied met specifications. <br />Ground Engineering conducted field testing and sampling of CP concrete as per specifications, and <br />will supply strength results at the completion of 28-day testing. <br />Golder observed and documented excavation and remediation activities associated with the Phase N <br />underground working construction. Full time monitoring was given to excavation of underground <br />workings. <br />Golder observed and documented confirmation drilling activities associated with the Phase IV <br />construction. Full time monitoring was given to confirmation drilling of underground workings <br />including logging holes, marking drill locations, surveying holes, and documenting underground <br />workings encountered. <br />Laboratory Test Results <br />Golder obtained QA samples SFa-09, SFb-04, SFb-O5, and SFb-06 of Structural Fill being placed in <br />Phase IVa and Phase IVb. Structural Fill samples SFb-OS and SFb-06 failed to meet the <br />specification for material passing the 200 sieve. Test results are presented in Attachment B.
