Phase I 1RE (continued) <br />If the Phase I THE fails eo identify a probable toxicant and/or a <br />probable source of toxicity, the permictee shall pacify the Division of <br />ies findings. She Division will determine whether a Phase II THE Ss <br />applicable. <br />If toxicity disappears during the Phase L TRE, the permictee shall <br />auhzlt written notlEicacion to that effect to the Division. If no acute toxicity Is <br />found to exist, the toxicity incident is closed. <br />7. Phase Ii THE (Out°alls 002, 011 and 012) <br />If a Phase II TR°_ is required, then .the following conditions apply: <br />A plan far a Phase II THE shall be submitted to the Division by the <br />permictee within 60 days of notification from the Div±sion that a Phase <br />II TiZE will be required. The PTt.se II T&E, as suiaitted to or revised <br />by the Division, shall be Sacorporated Snto the perzlc. <br />r'` <br />~` <br />The Phase II THE plan shall include the scope, methods, procedures, ~. <br />reporting requirements, and schedules. A Phase II TEtE may follow either <br />of two directions, as appropriate: toxicant treatability studies, or <br />source identification and control studies. Toxicant treatability studies <br />ors designed to assess the feasibility of eliminating S7'T via addition or <br />modification of [reatment processes ac the wastewater treatment facility. <br />Source identification and control efforts are designed to identify <br />toxicants, identify the sources of to xieants, and control whole effluen[ <br />toxicity by eliminating the introduction of toxicants into the <br />wastewater treatment facility. The Phase II Tfic permit conditions may <br />be amended, Sf so requested and justified by the permictee. <br />The Phase II TcZE report shall indicate the results of the Phase II THE <br />study. If the Phase II TBc. results in the identification of the <br />toxicants and the sources of toxicants, the permictee shall submit for <br />Division approval a control program to control STET within 60 days of <br />discovering the cause(s) of effluent torlclty, and shall proceed to <br />implement such plan within 7 days following ies approval by the DS vision. <br />If to xieicy disappears during the Phase TI TRE, the permictee shall <br />suhait written notification to that effect to the Division. The <br />Division may require the permictee to conduct accelerated testing Co <br />demonstrate the continued absence of toxicity. If no acute toxicity Ss <br />found to exist, the toxicity incident is closed. <br />~~ <br />
