Operation <br />Spill Preveruion Control and Countermeasure Plan Paee 4 <br />• corrective action taken or planned to be taken; <br />• name and telephone number of individual reporting the spill; and <br />• any additional pertinent information. <br />' Facilities that spill in excess of 1,000 gallons in a single spill event or have two spill events <br />within a 12-month period into or upon "navigable waters of the United States" must contact <br />the EPA and submit a copy of the SPCC plan as well as other information for review by EPA. <br />' Spill reporting at the B&B facility will be directed to the Safety Officer of the B&B facility. <br />It will be the Safety Officer's responsibility to call the National Response Center, if necessary. <br />A Spill Documentation Form, as shown in Attachment B, will be filled out and either submitted <br />' to the regulatory agencies or filed, as appropriate, depending on the severity of the spill. <br />Visible oil leaks which result in a loss of oil from tank or piping seams, gaskets, rivets and <br />' bolts sufficiently large to cause the accumulation of oil in the containment berm will be <br />promptly corrected. <br />' FACILITY INSPECTION AND DOCUMENTATION <br />The B&B facility will complete inspections every three months (quarterly) to verify berm <br />' integrity and changes with AST storage/usage. The inspector will be familiar with this plan. <br />Any changes from this SPCC plan will be noted. Upgrades will be made where needed, and <br />addendums to the SPCC plan will be made, if necessary. The inspection form shown in <br />' Attachment C will be filled out and filed after each inspection. This plan will be made <br />available to all personnel at the Montgomery Pit as a training tool for spill prevention at the <br />site. <br />SPILL PREVENTION PROVISIONS <br />At the time this SPCC plan was written, no spill events have occurred within the last year at <br />the B&B plant site. Past experience at the Montgomery Pit is that, in the course of routine <br />' operations, there have not been significant oil spill problems associated with the B&B facility. <br />Any spill event would be considered an unusual event at the B&B facility and would be <br />handled with appropriate care. <br />' Potential tank failures could occur due to possible seam failure, filtering failure, vandalism, <br />or valve failure. Other potential failures could be caused by tank overfilling, corrosion leaks, <br />' vehicle overfilling, and hose failure. None of these potential spill scenarios has been a <br />significant problem at the Montgomery Pit, but they are listed here to identify some of the <br />more common problems AST owner operators may have to deal with. Spills from any of the <br />' previously-described failures would be contained in either an AST berm or the respective basin <br />perimeter berm. <br />
