3) Appropriate title (CONDITIONAL USE PERMIT FOR Owner's Name or Proiect Name ); <br />4) Appropriate sub-title (brief description of the proposed use); <br />5) Legal description, describing the boundary of the property to be included in the use permit; <br />6) A vicinity map locating the development in relation to the surrounding areas, roadways, streets <br />and commonly known natural features (such as rivers, mountain peaks, cliffs, etcetera); <br />7) The drawing shall clearly label or show by identified line symbol, the boundary of the property <br />to be included in the use permit; <br />8) A North Point; <br />9) Written and graphic scale; <br />10) Date of prepazation and date of revision, if necessary; <br />11) The existing zoning classification for all properties adjoining the property to be included in the <br />use permit; <br />12) The size and location (by dimension) of all existing and proposed buildings and / or structures <br />to be housed on the subject property after issuance of the use permit; <br />13) The size and location (by dimension) of all off-street pazking and loading azeas on, or adjacent <br />to the subject property; <br />14) The size and location (by dimension) note statement and / or typical drawing of all off-street <br />pazking spaces; <br />I S) The size and location (by dimension) of the subject property's access to a public right-of--way; <br />16) The location of all drainageways or other natural features which effect the proposed use or <br />could have an effect due to the subject property; <br />i 7} The size and location (by dimension) of all pedestrian walkways and azeas; <br />18) The size and location (by dimension) of all service and refuse areas; <br />19) The size and location, by dimension and beazing, with labeling as to use and recording <br />information, of all existing and proposed easements. <br />c. A list identifying all agencies of local, state and / or federal government which will require a <br />permit, license or the like to be issued for all or part of the activity which comprises the proposed <br />use and a summary of the status of all applications, along with copies of such applications, <br />permits, licenses or the like and contact names at the agencies, packaged as one item and <br />marked as EXIIIBIT 47c.1 (NOTE: Any operation that requires a Colorado Department of <br />Natural Resources, Division of Minerals and Geology Reclamation Permit must provide copies <br />of the mining and reclamation plans [hat are submitted to the Division to obtain said permit). <br />d.
