<br />The State of Colorado, Division of Minerals and Geology requires that the operator of <br />any sand and gravel operation provide agreements between the operator and the owner <br />of any significant, valuable man-made structure located with 200 feet of our sand and <br />gravel operations know as THE SPECIAL 111 HIGHWAY 50 BORROW AREA NO. 2 <br />OPERATION. <br />The US West, 2524 Blickman, Grand Junction Colorado 81501, (970} 244-4411 owns <br />and maintains the following man-made structures within, or 200 feet of the above <br />indicated operations: <br />1. Underground Communication Line and Appurtenances <br />-CERTIFICATION - <br />The undersigned, Harold F. Elam, President of Elam Construction, Inc. does hereby <br />certify that the US WEST will be compensated for any costs incurred resulting from <br />damage to the above listed structures located within, or 200 feet of the affected land <br />described within Exhibit A, of the Construction Materials Application, as a result of our <br />mining activities. <br />ELAM ST TI C. <br />Harold F.
