<br />The State of Colorado, Division of Minerals and Geology requires that the operator of <br />any sand and gravel operation provide agreements between the operator and the owner <br />of any significant, valuable man-made structure located with 200 feet of our sand and <br />gravel operations know as THE SPECIAL HIGHWAY 50 BORROW NO. 2 <br />OPERATION. <br />The Grand Valley Rural Power Lines Inc, 2727 Grand Avenue, Grand Junction, <br />Colorado 81501, (970)242-0040 owns and maintains the following man-made structures <br />within, or 200 feet of the above indicated operations: <br />1. 7-Utiltiy Poles, Overhead Electric Lines and Appurtenances <br />-CERTIFICATION - <br />The undersigned, Harold F. Elam, President of Elam Construction, Inc. does hereby <br />certify that the GRAND VALLEY RURAL POWER LINES, INC. will be compensated for <br />any costs incurred resulting from damage to the above listed structures located within, <br />or 200 feet of the affected land described within Exhibit A, of the Construction Materials <br />Application, as a result of our mining activities. <br />ELAM ST 10 ,INC. <br />Harold F.
