<br />The State of Colorado, Division of Minerals and Geology requires that the <br />operator of any sand and gravel operation provide agreements between the <br />operator and the owner of any significant, valuable man-made structure located <br />on, or within 200 feet of our sand and gravel operations know as the THE <br />SPECIAL 1 i 1 HIGHWAY 50 BORROW AREA NO. 2 OPERATION. <br />The Colorado Department of Transportation, (CDOT) 222 South 6"' Street, Grand <br />Junction, Colorado 81501, owns and maintains the following man-made <br />structures on, or within 200 feet of the above indicated operations: <br />1. US Highway 50 and Appurtenances. <br />2. 2 -Drainage Culverts <br />-CERTIFICATION - <br />The undersigned, Harold F. Elam, President of Elam Construction, Inc. does <br />hereby certify that CDOT will be compensated for any costs incurred resulting <br />from damage to the above listed structures located within, or 200 feet, of the <br />affected land described within Exhibit A, of the Construction Materials <br />Application, as a result of our mining activities. <br />ELAM C ST <br />ar Id F.
