incorporated into the blasting plan to prevent damage. Copies of the report shall <br />be provided to the person requesting the survey and the Division. If the person <br />requesting the survey disagrees with the results of the survey, he or she may <br />notify, in writing, both the permittee and the Division of specific areas of <br />disagreement. <br />In this preblast survey, Chris Steven Russell, Professional Structural Engineer for AIC <br />conducted a survey of the structures and Walsh Environmental Scientists and <br />Engineers, LLC conducted a survey for the water wells. <br />Structural Evaluation <br />TRI-STATE G&T Assoc., Inc. PROPERTY <br />Tri-state G&T Association, Inc. requested that their power pole foundation pads near the <br />blast site be evaluated. Eight power poles were evaluated over a distance of 1 3/ miles. <br />These locations are noted on Pre-Blast Survey Locations drawing (DWG NO. 4.08.2-1, <br />Appendix). Power poles #3, #4, #5, and #6 are within 1000 feet of the blasting area. <br />Power poles #1,#2, #7 and #8 are more than a 1000 feet from the blasting area. Power <br />poles #1 and #8 are more than one-half mile away from the proposed blasting sites. <br />. Each of the 345KV power poles has two legs. Each leg is supported by a large diameter <br />concrete pier. The depth of each foundation is unknown. Photographs were taken of <br />each of the 16 concrete piers. It was found that some of the concrete piers were <br />cracked (Photo #2) but not to the extent of structural failure. One pier appeared to have <br />been subject to freezing whereby the edge was flaking off (Photo #10). Of course <br />concrete that has been subjected to freezing is weakened. Overall the concrete piers <br />appeared to be structurally sound. It is perceived that the blasting will not affect these <br />massive piers. However, they may be periodically checked to ascertain their current <br />condition. <br />PAUL ROUTSEN PROPERTY <br />6647 MOFFAT COUNTY ROAD 51 <br />Two buildings were evaluated at this location. No other buildings were noted here. <br />These two buildings include a modular home placed over a basement and a metal <br />building set over a concrete slab. <br />4 <br />
