Introduction <br />Allied Independent Consultants-Grand Junction, Inc. (AIC) was retained by Walsh <br />Environmental Scientists and Engineers, LLC to conduct a preblast survey for the <br />following property's structures: <br />TRI-STATE G&T Assoc., Inc <br />' (8) 345 KV POWER POLE FOUNDATIONS <br />PAUL ROUTSEN PROPERTY <br />6647 MOFFAT COUNTY ROAD 51 <br />7072 MOFFAT COUNTY ROAD 51 <br />The Colorado Division of Minerals and Geology define the scope of the preblast survey. <br />Their regulations state that : ~ <br />4.08.2 Pre-blasting Survey <br />(7) At least 30 days prior to initiation of blasting, th operator shall provide written <br />notification to all residents or owners of dwellinigs or other structures located <br />within one-half mile of the permit area which explains how to request a <br />• preblasting survey. Upon request to the Division by a resident or owner of a <br />dwelling or structure that is located within one-half mile of any part of the permit <br />area for surface mining activities, or in the case of surface blasting activities <br />incident to underground mining activities, within one-half mile of such blasting <br />activity, the person who conducts surface or underground mining activities shall <br />promptly conduct apre-blasting survey of the dwelling or structure and promptly <br />submit a report of the survey to the Division and to the person requesting the <br />survey. If the structure is renovated or added to, subsequent to a preblasting <br />survey, then upon request to the Division a survey of such additions and <br />renovations shall be performed in accordance with 4.08.2. <br />(2) The survey shall determine the condition of the dwelling or structure and <br />document any preblasting damage and other physical factors that could <br />reasonably be affected by the blasting. Assessments of structures such as <br />pipes, cables, transmission lines, and wells and other water systems shall be <br />limited to surface conditions and readily available data. Special attention shall be <br />given to the pre-blasting condition of welt and other water systems used for <br />human, animal, or agricultural purposes and to the quantity and quality of the <br />water. <br />(3) A written report of the survey shall be prepared and signed by the person who <br />conducted the survey. The report may include recommendations of any special <br />conditions or purposed adjustments to the blasting procedure which should be <br />
