<br />reporting of the incident will be made as soon as <br />practical to the appropriate agencies. <br />The reporting procedures are as follows: <br />a. The person detecting the spill will report the <br />spill to the shift foreman. <br />b. The shift foreman will notify the General <br />Manager who will designate a supervisor to <br />report the spill (see Appendix A for names and <br />address of Personnel. <br />c. The designated supervisor will report the event <br />to the federal, state, and local agencies as <br />listed above and in Appendix B. The supervisor <br />will document the date, time, and name of <br />person(s) contacted at each agency each time an <br />agency is notified of an incident (see Appendix <br />D: Forms - Table 1: Spill Incident Report to <br />File). <br />d. The information to be reported in the event of <br />;~ a spill includes: <br />1. Time and place of spill. <br />2. Source of spill. <br />3. Type and amount of material spilled. <br />4. Present location of spill and direction of <br />travel. <br />5. Known health hazards posed by the spill. <br />6. Weather conditions that would affect the <br />clean-up of the spill. <br />7. Action taken to initiate clean-up of <br />spill. <br />8. Any factual information requested by <br />agencies. DO NOT SPECULATE AS TO THE <br />CAUSE OR POTENTIAL IMPACT OF A SPILL. <br />iii. Description of Preventative Facilities for Spills <br />The Eastside Mine facility is designed to contain the <br />discharge of dieselJoil spills resulting from storage <br />equipment failure. The containment structures will <br />prevent spills from reaching the state water course in <br />the Harvey Gap Drainage. <br />a.
