<br />4) The Water Quality pond described on page G-1 is <br />included on Exhibit C, but none of the diversion ditches <br />in the process area which will lead to it are drawn on <br />the maps. Please include these, along with the designs <br />for any structures meant to handle water flows. <br />B) Exhibit D, Mining Plan: <br />1) The depth to groundwater has been omitted from the <br />application. Please submit an estimated depth to the <br />water table. Please demonstrate how this depth was <br />obtained. <br />2) Materials and application specifications for the <br />lining of Augmentation Pond #1 must be submitted prior to <br />construction of the pond. Please also include an <br />adequate Quality Assurance/Quality Control plan for its <br />construction, as well as design drawings. <br />C) Exhibit E, Reclamation Plan: <br />1) What is the thickness of overburden to be replaced? <br />2) Asphalt can be used as fill material as long as it <br />has been cured for more than 60 days. An explicit <br />commitment to the maximum amount of stockpiled asphalt <br />material at any one of time must be submitted, as well as <br />plans for reclaiming any excess left at the site. <br />3) Groundwater monitoring must be done at the site on a <br />regular basis, since this pit is in the alluvial plain of <br />the Blue River. Please submit monitoring schedules and <br />parameters, locations of monitoring wells, and baseline <br />data for groundwater monitoring. <br />4) Specifics of the seed mix application--- rate of <br />seeding for each species, and SCS fertilizer <br />requirements---have been omitted. Please submit this <br />information, including more specific estimates on shrub <br />and tree cuttings to be planted. <br />5) Please submit plans for a spill prevention and <br />materials containment for the asphalt and concrete batch <br />plants. <br />D) Exhibit G, Water Information: <br />In response to paragraph 1 of the Water Information <br />requirements ("If the operation is not expected to <br />directly affect surface or groundwater systems, a <br />statement of that expectation shall be submitted"), the <br />applicant responded: "This will not be the case." <br />However, since dikes will be left, standpipes and an <br />asphalt batch plant will be constructed and used at the <br />site, and fuel, lubricants, waste oil, etc. will be <br />stored on-site, further information is required and <br />outlined below. <br />
