The completed applicatioq along with all required design specifications, drawings, <br />~ geology reports, etc., must be submitted to the local health department no later than <br />two (2) week prior to the Boazd of Health meeting at which consideration is <br />~. requested. <br />r <br />~ 3.07 Regulation of Systems Contractors <br />L A, No person shall engage in business as a systems contractor without possessing a valid <br />Gilpin County Systems Contractor License. The initial fee for a Systems Contractor <br />i. License shall be paid to the local health department at the time the license application <br />is submitted. Licenses shall expire on December 31 each year and an annual renewal <br />f fee shall be paid to the local health department. A license which lapses because of <br />'~ failwe to renew or is revoked shall be subject to the fees established for new licenses <br />upon re-application. Employees of a licensed systems contractor aze not be required <br />I^ to be licensed. For the purpose ofthis sectioq "employee" shall apply only to those <br />` workers for whom a licensed contractor conmbutes unemployment insurance, soaal <br />.~ security and worker's compensation. <br />.~' B. Application for a Systems Contractor License or renewal shag be made upon forms <br />A supplied by the local health department. Prior to the issuance or renewal of a license, <br /> the health officer or his/her designated representative may require the applicant to <br /> demonstrate adequate knowledge of these regulations. In accepting a license, a <br /> systems contractor shall agree that all work performed in constructioq installation, <br />~ alteration or repair of an individual sewage disposal system shall be in compliance <br /> with these regulations and with the conditions set out in the permit. <br />i~ <br />~ <br />C. A Systems Contractor License may be revoked for failwe to comply with these <br />~ regulations. Written notice of a proposed license revocatioq particularizing the <br />"? violations of these regulations, shall be served by the local department of heathh upon <br />~ the holder of the Systems Contractor License by certified mail, return receipt <br />~ requested. Revocation shall take place only after a hearing before the local board of <br /> health. The license holder shall be given not less than ten (10) days notice of the <br />~ hearing and may be represented at the hearing by council. <br /> <br />' 3.08 Regulation of Systems Cleaners <br />A. No person shall engage in business as a Systems Cleaner without possessing a valid <br />y Gilpin Coumy Systems Cleaner License. Employees of a validly licensed Systems <br />.~ Cleaner are not be required to be licensed.
