8. Drawing number or similar identification; <br />9. Legal description of property, and for whom the system is designed. <br />8. The type of systems and sites enumerated below shall be designed by a Registered <br />Professional Engineer, and a permit for such system shall be issued only after review <br />and approval by the local board of health: <br />1. Systems disposing of effluent into State surface waters; <br />2. System disposing of effluent upon the surface of the Bound or utilizing <br />wastewater treatment ponds; <br />3. Individual sewage disposal systems which service commercial, business, <br />institutional or industrial property, or multi-family dwellings and dispose of <br />other than domestiatype wastes or dispose of sewage effluent through other <br />than an absorption system or vault; <br />4. Experimental systems; <br />5. Constructed wetlands; <br />6. Systems whose design flow exceeds 3,000 gallons per day (average daily Bow <br />exceeds 2,000 gallons per day); <br />7. Where the system requires a sift station or sewage pumping system; <br />8. Where the soil percolation rate is slower than sixty (60) minutes per inch or <br />faster than five (5) minutes per inch; <br />9. Where the maximum seasonal level of the groundwater table, bedrock, or <br />unsuitable soil is less than four (4) feet below bottom of the absorption azea. <br />10. Where the ground slope is in excess of thirty (30%) percent; <br />11. Where the system will be located within the flood fiinge; <br />12. Where a system must be installed such that any portion of the distribution <br />matron is above original grade. <br />13. Where the local board of health determines the system or the cumulative <br />effects of systems in an area or subdivision may detrimentally affect water <br />quality and construction is required in accordance with criteria more stringent <br />than the minimum criteria in these regulations. <br />14.
