2,2 Investigations
Reported personnel doses which exceed 50% of
the applicable limit, or reports by the vendor
that a badge was received in an abnormal con-
dition (i.e., damaged, highly contaminated,
etc.), will be investigated and resolved be-
fore the does is recorded. Typical investiga-
tion steps may include:
2.2,1 A review of the individual's work history.
2.2.2 Investigative surveys (Section V, 2.2),
or review of previous investigative sur-
veys of the individual's known work areas.
2.2.3 A review of the individual's TLD badge
history (i.e. was it taken home, worn
while undergoing an X-ray, etc.).
3. Records and Audits
Vendor reported doses will he posted to individual
exposure history files. Quarterly, year-to-da.te
(penetrating and nonpenetrating) and life-to-date
(penetrating only) doses will also be maintained.
Any investigation made in accordance with Section
IV.2.2 will be documented and posted to the indi-
vidual's exposure history file.
A summary of all TLD badge results, investigations
of excessive or erroneous doses, and corrective
actions taken or recommended will be incorporated
into the Radiation Protection Status Report.
