2,2 Investigations <br />Reported personnel doses which exceed 50% of <br />the applicable limit, or reports by the vendor <br />that a badge was received in an abnormal con- <br />dition (i.e., damaged, highly contaminated, <br />etc.), will be investigated and resolved be- <br />fore the does is recorded. Typical investiga- <br />tion steps may include: <br />2.2,1 A review of the individual's work history. <br />2.2.2 Investigative surveys (Section V, 2.2), <br />or review of previous investigative sur- <br />veys of the individual's known work areas. <br />2.2.3 A review of the individual's TLD badge <br />history (i.e. was it taken home, worn <br />while undergoing an X-ray, etc.). <br />3. Records and Audits <br />Vendor reported doses will he posted to individual <br />exposure history files. Quarterly, year-to-da.te <br />(penetrating and nonpenetrating) and life-to-date <br />(penetrating only) doses will also be maintained. <br />Any investigation made in accordance with Section <br />IV.2.2 will be documented and posted to the indi- <br />vidual's exposure history file. <br />A summary of all TLD badge results, investigations <br />of excessive or erroneous doses, and corrective <br />actions taken or recommended will be incorporated <br />into the Radiation Protection Status Report. <br />11 <br />
