Following is a
list of concerns and questions which must be addressed in order for the
application to be approvable. Our comments follow the exhibit outline as
shown on page 3 of the 112 application,
General
(1) Please complete pages one and two of the enclosed 112 permit application,
sign and return to our office.
(2) An application fee of E250.00 has been submitted. The 112 permit
application fee is calculated as E50.00 plus E15.00 per acre for the
~ ~ first fifty acres, with fractions of an acre being counted as an acre.
Thus, the applicable fee is E350.00 (E50.00 plus E15.00 per acre for
twenty acres). Please submit an additional E100.00 application fee.
(3) To expedite the final permit review and ultimate approval, it would be
best to consolidate the two applications into one, clear and concise
document. This document should follow the outline given on page 3 of the
112 permit application.
Exhibit A - Legal Description (2.12(1))
6 ~ Please consolidate the legal descriptions so that Exhibit A describes both
pits, overburden and topsoil stockpile area and the north-south access road on
the east side of the pits, and the main entry road for each pit.
Exhibit B - Index Map (2.12(2))
d ~ Index maps previously submitted will be sufficient for Exhibit B.
Exhibit C - Mining Plan Map (2.12(3))
~GV„~4 (1) Since this area is heavily disturbed by mining operations, no pre~nining
~~,~~ map is required.
~,:i} (2) Please submit a map which clearly (see Rule 2.12 for map criteria) shows
~'~~ !,~ the locations of the permanent structures identified in Exhibit B-3 of
the 110 permit applications.
