Joseph Dorris, Operator Crystal Jack Plan of Operations -June 30, 2005 • Revegetation of all disturbed areas with native vegetation. Before reclamation bond is released, vegetation must be established 80% or better compared to the surrounding undisturbed azeas. Reclamation of the site will be measrsed by the Minerals Administrator, South Pazk RD. Road-Use, Construction and Reclama[i:on: • Limit temporary roads, track hoe trails and other soil disturbances to the minimum feasible number, width, and total length consistent with the purpose of specific operations. • The operator must monitor all temporary access roads (new and existing) For erosion that would impact riparian comdors. The operator must. maintain water bars, rolling dips and lead-out ditches. Use weed-free certified straw bales in lead-out ditches as needed. Silt fencing must be installed as needed. See engineer specifications in Appemiix E. • If trees felled during road constmction they must be placed on contour with the slope. All non-system roads utilized through mining operations must be reclaimed to near-natural conditions immediately following the completion of mining operations as follows: o Roads and related stmctures must be obliterated to provide adequate drainage o All culverts must be removed and a natural grade reestablished. o Obliterated roads, dismpted stream banks and stream crossings must be recontoured and revegetated. o When temporary roads intersect perennial streams, biodegradable, wildlife friendly erosion matting or other stabilizing material must be used to reduce sedimentation until vegetation is established. Dig Site Construction and Reclamation: • Avoid operations during or directly after periods of heavy rain or when soils are wet. If rutting caused by equipment or vehicles is observed, stop mechanical operations immediately. • The operntor would be required to reclaim all dig sites by filling with excavated material, recontouring, replacing topsoil and seeding. • All mining spoil will be separated into two distinct piles: 1) topsoil and 2) general overburden. Topsoil will be considered the uppermost four inches of mineral soil and any organic matter (e.g. litter or duff) above the mineral surface. The topsoil must be piled and clearly labeled. On all slopes greater than 10 percent, topsoil and overburden piles must have silt fencing effectively installed down slope of the piles. Mining spoils must be placed unmediately upslope of dig sites such that any eroded material would be captured by the excavation. • All dig sites above riparian zones (as defined in the 9A standards and guidelines and the Forest Riparian Inventory) will be accessed from upslope positions or along contour (when operator safety pemrits) to limit soil disturbance and raveling downslope and into riparian azeas. Vegetation and Noxious Weeds: • Reclamation seed mixes must include only native species, and must exclude crested wheat grass, smooth brome and yellow sweet clover, which are all potentially invasive species. When purchasing seed, the operator must obtain a writte n statement indicating that the seed being purchased is weed- free. • The operator will use the South Park District's dry/lower elevation seed mix, riparian seed mix or willow spikes (1 spike per squaze foot) as designated. See Appendix F a description of the required seed mixes. • The operntor will seed all recontoured dig sites, associated topsoil piles and reclaimed roads at the end of every season. • Seeding will be done in the fall, optimdly before the fast snowfall. Apply 40 Ibs of seed per acre. • Reclamation in forested areas may require hand tools to pull excavated material away from trees. Revisions made by: Wendy M.
