Reports of activities at the emergency shall be made by :~~e <br />communicators. <br />• LOG OF EVENTS <br />Depending on the scope of the emergency, a responsible .ers~n in <br />the dispatching section may be instructed to maintain a log of t~•=_ events. <br />This log should include the following: <br />1. Time and date of receipt of the first call. <br />2. Inmediate action taken. <br />3. Time when various personnel were notified and dispatched. <br />4. Time and type of action taken to alleviate emergency condition, <br />such as changing operating pressures, closing valves, stopping <br />off, etc. <br />5. Time conditions were considered under control. <br />• There are many items that might be included in such a log, cepending <br />on the type of emergency. Even though an action might appear to ~e <br />insignificant at the time, details might prove to be invalua5le e a <br />later date. Caution must be taken to make the log as accurate ar:i com- <br />plete as possible and to include all decisions and actions exercised. <br />(See attached Emergency Log, Exhibit 1). <br />Eh1ERGENCY CONDITION CHECKLIST AND CHECKLIST FOR SUPERVISORS <br />During an emergency, it is difficult to think of all actions to be <br />considered in order to alleviate the problem. because of this difficulty, <br />there are Emergency Condition Checklists attached, see Exhibit (1) for <br />the supervisor in charge to use during the course of an emergency to <br />ascertain that all items on the checklists have been considered and <br />• <br />-10- <br />
